25 It may not be necessary <strong>for</strong> the persons who have already submitted such statementsduring the earlier revision and who do not desire to amend any particulars in theirstatement to send fresh statements, unless the <strong>Commission</strong> so directs in respect of anyrevision (like in case of de-novo preparation).26 Two copies of the Form (as applicable) have to be submitted. These Forms (2, 2A or 3as the case may be) should be filled up by the individuals concerned and handed over tothe officer-in-charge of each record office/unit or the nodal authority in MEA (asapplicable).27 To be enrolled at two places is an offence under the election laws. There<strong>for</strong>e everyForm 2 or 2A (not <strong>for</strong> Form 3) shall be accompanied with a declaration in a prescribedFormat (Annexure 10.2) to the effect that he did not get enrolled as ordinary lector inany constituency. This declaration need not be in duplicate.28 The officer in-charge at the address given in column 4 of the elector detail table (above)or the MEA nodal authority will check the Forms (and the Declaration) and ensure thatparticulars given by the applicants are correct and ensure that invariably the full addressincluding hometown or village and district is filled in.29 The service personnel have been instructed to give full particulars of their address sothat first, the district to which the Form has to be sent is easily identified, and secondly,that district authority i.e. the District <strong>Election</strong> Officer who normally is the DistrictCollector can easily determine which constituency the elector belongs to. Actualresidence is, of course, not necessary and so long as it gives adequate detail of hisprobable place of residence so as to enable the DEO/ERO to determine hisconstituency, the statement should be accepted.30 The officer-in-charge, after careful verification, shall sign the verification certificateprovided in the Form itself.31 The officer-in-charge will thereafter, bunch together all duly filled applications (induplicate) along with declarations attached, pertaining to a state and collectively<strong>for</strong>ward them to the CEO of the State mentioned in the address by the applicant (inForm). While <strong>for</strong>warding these Forms to a CEO, a covering letter with a list induplicate, of statements being <strong>for</strong>warded, should be sent. ( Annexure 10.3). A listshowing addresses of CEOs is <strong>for</strong>warded to MEA, MHA and MoD, every time the<strong>Commission</strong> undertakes revision of service voters.32 The CEO must acknowledge the receipt of these Forms by returning one copy of thestatement after preliminary checking of received <strong>for</strong>ms, to the sender. For facilitatingthis acknowledgement, the sending officers in MoD/MEA etc. must give their full andcomplete postal address on the communication. The ECI will quarterly monitor thereceipt and processing of service voter applications.33 Simultaneously, CEO will identify the DEO and send the Forms to the respective DEOs<strong>for</strong> further processing.81
