(v)(vi)(vii)(viii)(ix)(x)(xi)claimant is not in possession of any documentary proof of age, the ERO shouldguide the claimant to attach a declaration in a prescribed <strong>for</strong>mat (Annexure 5.6)by either of the parents, already included in the electoral roll, in support of the ageof their ward. In order to facilitate the registration of trans sexual in electoral rolls,the <strong>Commission</strong> has decided that a Guru in respect of his/her Chela in the agegroup of 18-25 years having no age proof, can make a declaration in the prescribedFormat (Annexure 5.6).If the applicant has an EPIC already issued to him, the EPIC detail must also becaptured in the Form 6. In that case, he should also provide photocopy of theEPIC. Non EPIC holders of 25+ age-group should give a declaration in prescribed<strong>for</strong>mat (Annexure 5.7)In part II of the Form 6, the complete address of ordinary residence where theapplicant desires to be enrolled has to be given. The applicant may be advised toattach a copy of any document like Bank/Kisan/Post Office Pass Book, RationCard, Driving License, Passport, Gas Connection, Telephone/Electricity/ WaterBill etc. showing that address. In case of non-availability of any document theDesignated Officer /BLO may receive the application and record thereon “Nodocument attached”. (In such cases local enquiry shall be got conducted by theERO and decision taken in accordance.)In cases where the applicant mentions the period of stay at that address <strong>for</strong> a longtime, say a year or more, then the circumstances because of which he did notapply <strong>for</strong> enrolment should be ascertained and recorded on the application <strong>for</strong>mby the designated officer.The applicant is required to indicate in Part III of Form 6, the details of his/herimmediate family members, if any, already included in the draft electoral roll. Incase the applicant has only indicated the name(s) of immediate family membersbut has not given other electoral roll details like part no., serial no. and EPIC No.of such family member(s), then the applicant should be advised to fill up thesedetails from the copy of the draft roll made available at each designated location;The applicant must fill in part IV of Form-6 full details of previous enrollment.However, failure to furnish the voter serial number, part number and theconstituency details, if not known, would not be a ground <strong>for</strong> non-acceptance ofapplications, if the previous address has been fully given. However, the previousaddress must be fully provided failing which the application shall not be received.In case the person has been living at a place <strong>for</strong> long and has not shifted residenceat all but still doesn’t have elector details, the person should be advised to fill inthe complete present address in the space provided <strong>for</strong> previous address.The most important part of the preliminary checking is to see that the Declarationin Part IV, at the end of the Form-6 is complete in all respects. From applicants ofage 25+, the previous address of ordinary residence must be obtained anddatabase search done to ascertain that the applicant is not already enrolledelsewhere and delete his/her name from the electoral roll of the other placeonce the name is included in the present place.Applicant, if he is service personnel, must enclose the additional Declarationstating non-inclusion of his name as a service elector in the last part of theelectoral roll of his native place.23.
