APPENDIX B: STATEMENT OF WORK<br />

1.0 SCOPE OF WORK OVERVIEW<br />

Contractor shall provide all personnel, space, telephone services, utilities,<br />

equipment, vehicles, services, supplies, forms <strong>and</strong> any other items necessary to<br />

maintain control <strong>and</strong> accountability to provide necessary services described in<br />

Section 5.0, Specific Tasks, hereunder. In addition, Contractor shall add any<br />

additional category of cases, records or documents the County deems necessary<br />

<strong>and</strong> provide storage <strong>and</strong> pick-up <strong>and</strong> delivery as directed by County. Contractor<br />

must perform to the st<strong>and</strong>ards set forth in SOW Technical Exhibit B-1, Performance<br />

Requirements Summary, hereunder.<br />

1.1 Contractor Personnel<br />

A. Contract Manager<br />

The Contractor will provide a Contract Manager <strong>and</strong> alternate who will<br />

act as primary liaison with DPSS <strong>and</strong> be responsible for the overall<br />

management <strong>and</strong> coordination of this Contract. The Contract<br />

Manager <strong>and</strong> alternate shall be identified in writing prior to Contract<br />

award <strong>and</strong> at any time thereafter a change of Contract Manager or<br />

alternate is made. Specifically, the Contractor Manager, or his/her<br />

alternate, shall:<br />

• Have full authority to act for the Contractor on all Contract matters<br />

relating to the daily operation of this Contract.<br />

• Be available between 8:00 a.m. <strong>and</strong> 5:00 p.m., Monday through<br />

Friday, except County holidays.<br />

• Be able to read, write, speak, <strong>and</strong> underst<strong>and</strong> English fluently.<br />

B. Other Contractor Personnel<br />

The Contractor will:<br />

Identify, under sworn statement, all Contractor employees who are<br />

receiving public assistance <strong>and</strong> ensure that any employee receiving<br />

public assistance has met his/her reporting responsibility to the<br />

County <strong>and</strong> has no access to County <strong>and</strong> Contractor records of any<br />

friends, relatives, business relations, personal acquaintance, tenants,<br />

or any individual whose relationship could reasonably sway his/her<br />

conduct or performance on the job. Access includes, but is not limited<br />

to, determining eligibility for public assistance, transmitting computer<br />

data, <strong>and</strong> physical possession of case documents.<br />

• Be responsible for removing <strong>and</strong> replacing, within 24 hours, any<br />

Contractor employee performing services under the Contract <strong>and</strong><br />

insuring that such individual’s duties are satisfactorily performed<br />
