APPENDIX B: STATEMENT OF WORK<br />

by Contractor shall be signed for by an authorized County<br />

employee on a Contractor provided delivery manifest. Special<br />

Investigation <strong>Record</strong>s may only be requested <strong>and</strong> received by<br />

authorized personnel at Welfare Fraud Prevention & Investigations<br />

<strong>and</strong> by authorized personnel at the Office of the District Attorney,<br />

<strong>and</strong> shall not be delivered to any other County location, unless<br />

Contractor is specifically directed to do so by the County Contract<br />

Administrator.<br />

4.3.8.1 The signed delivery manifest, listing <strong>Record</strong>s requested,<br />

retrieved <strong>and</strong> delivered shall be the basis for the monthly<br />

invoice from Contractor.<br />

4.3.8.2 <strong>Retrieval</strong> Request Forms <strong>and</strong> all documentation of<br />

telephone calls from County authorized personnel<br />

requesting emergent <strong>Record</strong> retrievals shall be<br />

maintained to support the <strong>Record</strong>s listed on the delivery<br />

manifest.<br />

4.3.8.3 <strong>Retrieval</strong> Requests shall be in writing <strong>and</strong> shall come<br />

from County offices or special functions as listed in SOW<br />

Technical Exhibit B-7 Requests from other than those<br />

County offices or special functions listed in SOW<br />

Technical Exhibit B-7 shall be sent to Contractor by the<br />

CCA. County may periodically update the County offices<br />

<strong>and</strong> special functions listed in SOW Technical<br />

Exhibit B-7.<br />

4.3.9 <strong>Retrieval</strong> Process<br />

Contractor shall locate <strong>and</strong> pull each <strong>Case</strong> <strong>Record</strong>/Special<br />

Investigation <strong>Record</strong> for which a <strong>Retrieval</strong> Request Form has been<br />

received <strong>and</strong> update Contractor’s Inventory System to show that<br />

the <strong>Record</strong> has been Borrowed or Withdrawn, the number of the<br />

County office which requested it, the date of the request, whether it<br />

is a Regular <strong>Retrieval</strong> Request or Emergent <strong>Retrieval</strong> Request,<br />

<strong>and</strong> the date the <strong>Record</strong> was sent out for delivery.<br />

A retrieved <strong>Case</strong> <strong>Record</strong>/Special Investigation <strong>Record</strong> shall be<br />

treated as either Borrowed or Withdrawn as designated by County<br />

on its <strong>Retrieval</strong> Request Form. In the event no such designation is<br />

made by County, the <strong>Case</strong> <strong>Record</strong> shall be considered Withdrawn.<br />

4.3.9.1 Data Entry<br />

Contractor shall perform data entry for all <strong>Record</strong>s listed<br />

on the DPSS <strong>Retrieval</strong> Request Forms unless the<br />

retrieval is rejected because it is illegible, inaccurate or<br />

incomplete.<br />
