APPENDIX A: SAMPLE CONTRACT<br />

2.14 Contract<br />

Agreement executed between County <strong>and</strong> Contractor. It sets forth<br />

the terms <strong>and</strong> conditions for the issuance <strong>and</strong> performance of the<br />

Statement of Work, (Attachment A).<br />

2.15 Contract Discrepancy Report<br />

A report used by the Quality Assurance Evaluator to record<br />

Contract information regarding discrepancies or problems with<br />

Contractor’s performance. If Contractor’s performance is<br />

determined to be unsatisfactory, the CCA is required to forward the<br />

CDR to Contractor for his response (See SOW Technical Exhibit B-<br />

2).<br />

2.16 Contract Management Division<br />

The Department of Public Social <strong>Services</strong>’ Division responsible for<br />

the Contract.<br />

2.17 Contractor<br />

The sole proprietor, partnership, or corporation that has entered into<br />

a contract with the County to perform or execute the work covered<br />

by the Statement of Work.<br />

2.18 Contractor Contract Manager<br />

The individual designated by the Contractor to act as a liaison with<br />

the County <strong>and</strong> be responsible for the administration of the Contract<br />

operations after the Contract award.<br />

2.19 County Contract Administrator (CCA)<br />

Person with responsibility to oversee the day-to-day activities of this<br />

Contract. Responsibility for inspections of any <strong>and</strong> all tasks,<br />

deliverables, goods, services, <strong>and</strong> other work provided by<br />

Contractor.<br />

2.20 County Contract Program Monitor (CPM)<br />

County personnel under the supervision of the County Contract<br />

Administrator who is responsible for monitoring the Contractor’s<br />

compliance with the contract provisions. The CPM is responsible for<br />

inspections of any <strong>and</strong> all tasks, deliverables, goods, services <strong>and</strong><br />

other work provided by the Contractor.<br />
