PROPOSAL SUBMISSION REQUIREMENTS<br />

box, plainly marked in the upper left-h<strong>and</strong> corner with the name <strong>and</strong><br />

address of the Proposer <strong>and</strong> bear the words:<br />

“COST PROPOSAL FOR CASE RECORD STORAGE AND RETRIEVAL<br />

SERVICES <strong>RFP</strong> CMD# 12-06"<br />

It is the sole responsibility of the submitting Proposer to ensure that its<br />

proposal is received on or before the submission deadline. Submitting<br />

Proposers shall bear all risks associated with delays in the U.S. Mail.<br />

3.11 Preparation of the Proposal<br />

Two separate proposals must be submitted - a Business Proposal <strong>and</strong> a Cost<br />

Proposal. All Proposals must be bound <strong>and</strong> submitted in the prescribed format.<br />

Any Proposal that deviates from this format may be rejected without review at<br />

the County’s sole discretion.<br />

In preparing the written proposal, the Proposer should ensure that the proposal<br />

responds completely <strong>and</strong> thoroughly to all requirements set forth in this <strong>RFP</strong>.<br />

The objective of the proposal submission is for DPSS to ascertain the Proposer’s<br />

ability to meet or exceed the required service level. In addition, specific<br />

information is requested from all Proposers to ensure that the proposals can be<br />

fairly compared <strong>and</strong> evaluated in a st<strong>and</strong>ard manner.<br />

Only information that is contained in the written proposal will be evaluated.<br />

Proposals must adhere to the specified page limits. The County may, in its sole<br />

discretion, disregard any pages over the limits.<br />

The one original proposal must be labeled as “Original” on the cover page.<br />

Copies must be labeled on the cover page as “Copy 1 of 4,” “Copy 2 of 4,” etc.,<br />

as appropriate. The Proposal is written in English <strong>and</strong> all numerical data<br />

furnished are in foot, pound, <strong>and</strong> second system of units of measurement.<br />

Additionally, a minimum of four separate CDs, containing files in Adobe Acrobat<br />

7.0 Professional format, shall be labeled <strong>and</strong> submitted for each Proposal as<br />

follows:<br />

• Business Proposal (may be submitted on as many CDs to accommodate a<br />

complete Business Proposal)<br />

• Cost Proposal<br />

• Financial Statement<br />

• Documents/pages identified as trade secrets, proprietary <strong>and</strong>/or<br />

confidential, if none, so state.<br />

Proposers shall observe the requirements set forth in this Section 3.0, Proposal<br />

Submission Requirements, in the preparation of their proposal <strong>and</strong> shall agree to<br />

provide the County with any additional information necessary for an accurate<br />
