PROPOSAL SUBMISSION REQUIREMENTS<br />

.5 Contractor’s Transition Plans (Business Proposal, Section C.5)<br />

The Proposer must provide two comprehensive transition plans as<br />

follows:<br />

1. The first plan must explain the Proposer’s specific<br />

implementation plans, including but not limited to, plans<br />

for assumption of services from the predecessor, to<br />

ensure high quality services effective from the first day of<br />

operation. The implementation plan shall include, but not<br />

be limited to, staff recruitment, any acquisitions deemed<br />

appropriate by the Proposer <strong>and</strong> any non-County<br />

provided training.<br />

2. The second plan must explain how services will be<br />

transitioned at no cost to the County to another<br />

Contractor when the Contract ends. This plan must<br />

include a statement of cooperation should a transition to a<br />

new provider be necessary.<br />

3.12.8 Proposers Staffing Plan (Business Proposal, Section D)<br />

The Proposer must provide a detailed staffing plan that will ensure full<br />

compliance with the Contract’s requirements <strong>and</strong> the Proposer’s stated<br />

methodology of providing employment services. The plan must be<br />

supported by <strong>and</strong> consistent with the Proposer’s Budgeted Costs.<br />

In submitting a proposal, Proposers are asked to specifically address<br />

the manner in which they will ensure staffing needs are met for delivery<br />

of case record storage <strong>and</strong> retrieval as described in the Appendix B,<br />

Statement of Work.<br />

.1 Qualifications of Staff (Business Proposal, Section D.1) Limit to<br />

Five Pages<br />

1. The Proposer must identify <strong>and</strong> provide a resume for the<br />

Contractor’s Contract Manager who will be responsible for<br />

addressing staffing required service changes <strong>and</strong> monitoring this<br />

Contract. The Contractor’s Contract Manager will act as a liaison<br />

with DPSS <strong>and</strong> will be responsible for the overall management<br />

<strong>and</strong> coordination of this Contract. The Contract Manager’s<br />

resume <strong>and</strong> all resumes noted below will include, at minimum,<br />

the following:<br />

a. The current position <strong>and</strong> years with the firm.<br />

b. The academic background, covering all education at the high<br />

school level <strong>and</strong> above, includes the names of the<br />

institution(s) <strong>and</strong> dates of attendance.<br />
