PROPOSAL SUBMISSION REQUIREMENTS c. The experience background, including the number of years’ experience, position/titles, and functions, while gaining experience in the provision of the services to be delivered under the Contract. d. The Contractor’s Contract Manager must meet the minimum mandatory requirements outlined in RFP, Subsection, 1.5 Minimum Mandatory Requirements. If the Contractor’s Contract Manager is not hired by the Contract award date the Proposer must include with its proposal the complete job specifications for this position, including but not limited to the required number of years’ experience in the area of case records storage services, the minimum educational background, if any, and minimum management experience, if any. (Business Proposal, Section D.1.1) 2. Availability of Qualified Back-up Staff Similarly, the Proposer is to provide a resume for the required Back-Up Contract Manager who will assume the Contractor’s Contract Manager’s responsibilities in his/her absence. Include resumes with, at minimum, the criteria listed in this RFP Section 3.12.8.1.1 (a through d), above. If not yet hired, the Proposer must include with its proposal the complete job specifications for this position, including, but not limited to, the required number of years of experience in the area of case record storage and retrieval services, the minimum educational background, if any and minimum management experience, if any. (Business Proposal, Section D.1.2) 3. Proposer’s Management Staff Qualifications The Proposer must include a list of main management personnel who will be assigned in a management capacity on any part of this project, how each will be utilized in the management of the required services. Include resumes with, at minimum, the criteria listed in RFP, Section 3.12.8.1.1. (a through d), above. (Business Proposal, Section D.1.3) 4. Proposer’s Non-Management Qualifications The Proposer must include a complete list of all staff (by job classification/functional title, not by name); explain how the staff will be utilized to provide employment services. (Business Proposal, Section D.1.4) .2 Organizational Structure in Providing Services (Business Proposal, Section D.2) – Limit to Two Pages The Proposer must explain the organizational structure proposed in the Proposer’s plan. This structure should include a narrative explaining how it arrived at the structure 3.12.9 Proposer’s Quality Control Plan (Business Proposal, Section E) - Limit to Five Pages Present a comprehensive Quality Control Plan to be utilized by the Proposer as a self-monitoring tool to ensure the required services are provided as specified in Appendix B, Statement of Work and Appendix C, SOW Technical Exhibit C-1, Performance Requirements Summary Chart. The following factors may be included in the plan: • Activities to be monitored to ensure compliance with all Contract requirements; • Monitoring methods to be used; • Frequency of monitoring; • Samples of forms to be used in monitoring; • Title/level and qualifications of personnel performing monitoring functions; • Documentation methods of all monitoring results, including any corrective action taken; and • Plan to file all monitoring results, including any corrective action taken. 3.12.10 Proposer’s Green Initiatives (Section F) Present a description of proposed plan for complying with the green requirements as described in Section 5.0 of the SOW.
