lethargy for 2 weeks, having an arched back, anorexia, <strong>and</strong> diarrhea<br />

for 2 days. Survey radiographs revealed a large calculus in<br />

the right renal pelvis <strong>and</strong> the suggestion of a calculus in the left<br />

ureter. The excretory urogram revealed a severely dilated right<br />

renal sinus with a central filling defect that is shaped similarly to<br />

the calculus (black arrow). The ureter distal to the pelvis is normal.<br />

The left kidney revealed a dilated renal pelvis <strong>and</strong> proximal<br />

ureter (long white arrows) as well as diverticula. The ureteral<br />

dilation (small white arrows) ends at the level of the ureteral calculus<br />

(open white arrow). Diagnosis: Right renal calculus <strong>and</strong><br />

left ureteral calculus with resultant proximal hydroureter <strong>and</strong><br />

hydronephrosis (see Fig. 3-179).<br />

There is mild global enlargement<br />

of the left kidney <strong>and</strong> the<br />

collecting system of the kidney <strong>and</strong><br />

the entire length of the ureter (white<br />

arrows). The right kidney is quite<br />

small with no discernible collecting<br />

structures. There is a large filling<br />

defect in the trigone region of the<br />

bladder (black arrow). Differential<br />

diagnoses for the right kidney<br />

include congenital hypoplasia or<br />

chronic atrophic pyelonephritis,<br />

those for the left kidney include<br />

obstructive uropathy or acute<br />

pyelonephritis <strong>and</strong> ureteritis, <strong>and</strong><br />

those for the bladder include neoplasia<br />

or granuloma. Diagnosis:<br />

Transitional cell carcinoma of the<br />

bladder with obstruction of the left<br />

ureter <strong>and</strong> chronic atrophic<br />

pyelonephritis of the right kidney.
