Lung carcinoid<br />

- Neuro-endocrine tumor arising from Kulchitsky (K)cell [cells from amine precursor<br />

uptake and decarboxylation (APUD) system].<br />

- The vast majority of bronchial adenomas are carcinoid tumours<br />

- Lung carcinoid accounts 1% of lung tumours and for 10% of carcinoid tumours.<br />

• Features<br />

Typical age = 40-50 years<br />

Site : main bronchi ; most frequently in RT middle lobe .<br />

Smoking not risk factor<br />

Slow growing: e.g.<br />

- long history of cough, recurrent haemoptysis<br />

- intestinal cramps & diarrhoea<br />

Often centrally located and not seen on CXR<br />

Highly vascular : 'Cherry red ball' often seen on bronchoscopy (pathognomonic)<br />

Carcinoid syndrome itself is rare (associated with 5% of liver metastases).<br />

Bronchoscopy & biopsy<br />

- Bronchoscopy identifies up to 80% of carcinoid tumors in the main bronchi<br />

- Biopsy is usually followed with brisk bleeding and should be done via rigid<br />

bronchoscopy.<br />

• Management<br />

Surgical resection<br />

• Prognosis : if no metastases then 90% survival at 5 years.
