Viral hepatitis B<br />

- HBV is a double-stranded DNA virus.<br />

• Mode of transmission :<br />

1. Exposure to infected blood or body fluids, including vertical transmission from<br />

mother to child & sexual intercourse (breast feeding doesn`t transmit HBV).<br />

2. Risk of transmission if single needle brick is 20-30%<br />

3. Vertical transmission risk to baby 20% & ↑ to 90% if the mother is HBe AG +ve.<br />

4. The incubation period is 6-20 weeks.<br />

• Clinical presentation :<br />

Most of the cases are symptomotic.<br />

Acute hepatitis : more aggressive than HCV , may be fulminant LCF & death.<br />

Chronic hepatitis (in 5-10% of cases).<br />

Extrahepatic (due to immune complexes containing hepatitis antigens):<br />

o Glomerulonephritis : MPGN ,membranous GN.<br />

o PAN.<br />

• Investigations (HBV serology) :<br />
