Description<br />

Flat; any change in color of the skin < 1 cm in size<br />

Flat lesion > 1 cm in size<br />

Solid elevated lesion < 1 cm diameter; greatest mass above skin surface<br />

Solid elevated lesion > 1 cm diameter; greatest mass below skin surface<br />

Fluid-filled (clear) < 1 cm diameter, usually < 0.5cm<br />

Fluid-filled (clear) > 1 cm diameter<br />

Vesicle or bulla with purulent fluid<br />

Cavity lined with epithelium containing fluid, pus, or keratin<br />
