NB : Dipeptidyl Peptidase-4 (DPP-4) Inhibitors (e.g. Sitagliptin[januvia])<br />

Normal mechanism of insulin secretion & action of sitagliptin<br />

- During a meal the incretins [glucagon-like peptide 1 (GLP-1) and glucose-dependent<br />

gastric inhibitory polypeptide (GIP)] are released from the small intestine into the blood<br />

to ++ pancreas to secrete insulin & -- glucagon.<br />

- These incretins normaly are catabolised & inactivated by DPP-4 enzyme, so - - of DPP-4<br />

by sitagliptin will → no destruction of incretins &prolonged exposure of pancreas to<br />

incretins → ↑ insulin & ↓ glucagon → ↓↓ blood sugar.<br />

- NO hypoglycemia.<br />

- NO weight gain.
