• Types according to the types of circulating antibodies present :<br />

Type I Type II Type III<br />

ANA and/or<br />

(SMA) (anti-smooth<br />

muscle antibodies)<br />

Affects both adults and<br />

children<br />

(LKM1) (Anti-liver/kidney<br />

microsomal type 1 antibodies).<br />

Affects children only<br />

Soluble liver-kidney<br />

antigen.<br />

Affects adults in<br />

middle-age<br />

• Features :<br />

Chronic liver disease.<br />

Acute hepatitis: fever, jaundice etc (only 25% present in this way)<br />

Amenorrhoea (common)<br />

ANA/SMA/LKM1 antibodies, raised IgG levels.<br />

liver biopsy: inflammation extending beyond limiting plate 'piecemeal necrosis',<br />

bridging necrosis<br />

• Management :<br />

Steroids .<br />

Immunosuppressants e.g. azathioprine<br />

liver transplantation.<br />

Portal tracts show a variable number of small<br />

lymphocytes admixed with plasma cells<br />

(arrowheads).<br />

In contrast to primary biliary cirrhosis (PBC) bile<br />

ducts are intact (arrow).<br />

The type and amount of chronic inflammation does<br />

not differentiate AIH from viral hepatides.<br />
