• Main clinical features :<br />

Dyskinesia<br />

Hypokinesia: Difficult initiating movements (= Poverty of movement ) : mask face<br />

Bradykinesia : Slow movement : Shuffling gait<br />

Akinetic asymmetrical tremors<br />

Tremors<br />

- Most marked at rest (4-6Hz) : improve with moving the affected limb (holding something)<br />

- Typically 'pill-rolling'<br />

Rigidity :<br />

- Lead pipe rigidity :- Hypertonicity felt by examiner in a parkinsonian limb throughout the range of<br />

movements of a joint.<br />

- Cog-wheel rigidity : - Feeling of periodic resistance to passive movement felt by the examiner in a<br />

parkinsonian limb.<br />

- It`s a tremor superimposed on lead pipe rigidity - the teeth of the cog are<br />

analogous to the tensed parts of the tremor.<br />
