Evaluating Phlebotomy Practical Performance

Two forms are presented in Tables 11-4 and 11-5 for evaluating a student’s

performance in collecting blood. These forms are flexible by nature and can

be modified to ascribe points for proper performance and to record a grade if

desired. All of the essential steps in sequence are provided on these forms.

The students should review the entire pictorial presentation of Figure 11-17

before attempting a venipuncture, or use the photographs to assess any prior

attempt and to troubleshoot any problems encountered.
