ESSENTIALS OF CLINICAL LABORATORY SCIENCE

In the Lab

3. Place an Acetest tablet on a filter paper disc or other white absorbent paper.

4. Place 1 drop of urine directly on the Acetest tablet (Figure 12-35).

5. At 30 seconds, compare the color of the tablet with that on the chart provided by

the manufacturer.

6. Discard all supplies used for the procedure in the appropriate containers.

A disinfectant should be used to clean the work surfaces and equipment should

be cleaned and restored to its former position. Gloves should be discarded

appropriately and the hands washed thoroughly in accordance with established

policies.

Reporting of Results

Report on the Procedure Report Form or other appropriate form the semiquantitative

URINALYSIS AND BODY FLUIDS ANALYSIS PROCEDURE #4

Ictotest—Semiquantitative Test for Bilirubin in Urine

Principles

Bilirubin is a product derived from the breakdown of red blood cells. Extremely high

levels of bilirubin may be found in liver disease, where the liver is unable to form other

products from the excess hemoglobin, and in cases of high destruction of red blood

cells. Testing for the presence of abnormal levels of bilirubin in the urine is most often

performed by using Ictotest reagent tablets. When exposed to clinically significant levels

of bilirubin in the urine, these tablets react based on the coupling of a unique solid

diazonium salt with bilirubin in an acid medium, resulting in a blue or purple reaction

product. Mild colors, including pink, are not to be reported as positive bilirubin findings.

Pyridium is a medication sometimes used in cystitis (bladder infections) that will

mask positive results when small amounts of bilirubin are present. Other drugs (e.g.,

chlorpromazine or etodolac) may also cause a false positive result or atypical results.
