The following list of states and territories with laboratory personnel licensure and
contact information was updated as of January 2009. Consult Web sites for current
information regarding application and renewal fees as well as requirements for
continuing education. If not otherwise stated, annual renewal is assumed.

Beginning in 2008, Laboratory Field Services (LFS) no
longer administers the State-Administered Examination
for CLS (generalist) and for the three categories of
specialists (limited) licensing (clinical chemist, clinical
microbiologist, and clinical toxicologist scientist).
Applicants must select one license category as listed on
the preceding website. Applicants for specialty (limited)
license of chemistry, microbiology, and toxicology
can apply to take a California-approved certifying
organization exam (ASCP examination for chemistry
and microbiology is approved and NRCC examination
for toxicology is approved). Applicants for specialty
(limited) license of immunohematology and hematology
have to take a state administered examination.
