John McDonald, a mine boss, aged 47 years, was instantly killed<br />

by being struck by a loaded dilly trip, while it was ascending <strong>the</strong><br />

slope near <strong>the</strong> No. 2 flat left, in Mammoth shaft mine, on August 2.<br />

Just how <strong>the</strong> accident occurred is not known, as <strong>the</strong>re was no one<br />

present at <strong>the</strong> time. Mr. McDonald was for many years mine foreman<br />

at <strong>the</strong> Moyer mines, and left <strong>the</strong>re about four weekss previous<br />

to his death to accept <strong>the</strong> foremanship at <strong>the</strong> Mammoth mine.<br />

survived by a widow and nine children.<br />

George Dezeyak, a Slavish laborer, at <strong>the</strong> Alexandria mine, was<br />

smo<strong>the</strong>red to death in <strong>the</strong> coal bin, on July 14, by <strong>the</strong> coal sliding<br />

down on him. The coal is crushed and washed at this mine, which<br />

makes it very fine, causing it at time to adhere to <strong>the</strong> sides <strong>of</strong> <strong>the</strong><br />

bin, as was <strong>the</strong> case when <strong>the</strong> accident occurred.<br />

James McNaught, an American driver, aged 21 years and single,<br />

was instantly killed in Hempfield mine on August 7, by a fall <strong>of</strong> ro<strong>of</strong>.<br />

Owen Morgan, an Irish miner, aged 39 years, was instantlj<br />

killed<br />

by a fall <strong>of</strong> coal in Alexandria mine on August 13.<br />

Frederick Wilhelm, an American miner, aged 17 years, was seriously<br />

injured by a fall <strong>of</strong> slate in Arona mine, August 13. He was<br />

removed to <strong>the</strong> Mercy Hospital, at Pittsburgh, two days after <strong>the</strong><br />

accident and died <strong>the</strong>re on November 2.<br />

Marco Bacic, an Austrian miner, aged 27 years, employed in Larimer<br />

mine, was instantly killed by a fall <strong>of</strong> slate on August 27.<br />

Frank Piershe, an Austrian miner, aged 27 years and single, was<br />

seriously injured by a fall <strong>of</strong> slate on August 17, breaking his back<br />

and left ankle. He was removed to <strong>the</strong> Cottage Hospital, at Connellsville,<br />

where he died on September 10, twenty-four days after <strong>the</strong> accident.<br />

Elmer Shultz, an American, a night pumper, was injured by a piece<br />

<strong>of</strong> rock or wood which fell down <strong>the</strong> shaft and struck him on <strong>the</strong><br />

head, crushing his skull, as he was on <strong>the</strong> cage descending Hecla No. 1<br />

shaft, on <strong>the</strong> evening <strong>of</strong> September 8, about 6.30 P. M. He died September<br />

10, two days after <strong>the</strong> accident. On investigation. I discovered<br />

that <strong>the</strong>re was no overhead cover on <strong>the</strong> cage which <strong>the</strong> decedent<br />

and a man named Jeffrey was on when <strong>the</strong> accident occurred. 1<br />

was informed by <strong>the</strong> <strong>of</strong>ficials <strong>of</strong> <strong>the</strong> mine and o<strong>the</strong>rs that persons were<br />

not allowed to ride on this cage, it being used exclusively for lowering<br />

materials into <strong>the</strong> mine..<br />

Jeffrey stated that he knew it was against<br />

<strong>the</strong> rules <strong>of</strong> <strong>the</strong> mine to go down on <strong>the</strong> cage when ho and Shultz<br />

boarded it. The air shaft at this place is provided with a stairway for<br />

persons going into or out <strong>of</strong> <strong>the</strong> mine, when <strong>the</strong>y cannot use <strong>the</strong> cage<br />

which is supplied with an overhead cover. John W. Bailoy stated<br />

that Shultz traveled <strong>the</strong> stairway about two weeks while <strong>the</strong>y were<br />

engaged in putting new timber in <strong>the</strong> shaft. I found notices, properly<br />

signed and posted, at <strong>the</strong> top and bottom <strong>of</strong> <strong>the</strong> shaft, forbidding
