Der kræves ≥ 4/6 loci forlig mellem patient og navlesnorsblod. I tilfælde af at recipient skal have blod fra to navlesnore, skal de indbyrdes også matche ≥ 4/6. De indbyrdes 4/6 match behøver ikke nødvendigvis at være de samme match som til patients vævstype. Valg i prioteret rækkefølge: 1. Blod fra enkelt 6/6 match navlesnor med ≥ 3.0 x 10 7 TNC / recipient kg 2. Blod fra enkelt 5/6 match navlesnor med ≥ 4.0 x 10 7 TNC / recipient kg 3. Blod fra to navlesnore: • Hver navlesnor skal indeholde ≥ 1.5 x 10 7 TNC / recipient kg • Unit 1: Vælg unit med fleste HLA match ud fra units med doser ≥ 1.5 x 10 7 TNC recipient kg • Unit 2: Vælg unit med fleste HLA match ud fra de tilbageværende units med doser ≥ 1.5 x 10 7 TNC / recipient kg, med mindre der er en unit med 1 antigen dårligere match, som indeholder en celle dosis som er ≥ dobbelt så stor end den units med det bedre match. Yderligere udvælgelses kriterier: Hvis der er flere navlesnore med samme grad af HLA match til recipient og med ≥ 1,5 x 10 7 TNC recipient kg prioteres som følger: • 1. prioritet: Flest antal TNC • 2. prioritet: Hvis CD34+ tælling foreligger, og der findes to navlesnore med samme HLA match og samme TNC dosis (± 0.3 x 10 7 /kg), vælges den navlesnor med det største antal CD34+ celler. Hvis CD34 tallet ikke fremgår, kan det evt. efterspørges. • 3. prioritet: Indenfor samme grad af HLA match og samme indhold af TNC dosis (± 0.3 x 10 7 /kg) har match af DRB1 største prioritet. Vejledende kommentarer: Blod fra en navlesnor med 5/6 match, hvor donor er homozygot i det mismatchede locus bør foretrækkes fremfor blod fra en navlesnor med et 5/6 match i begge retninger. (Eksempel: Hvis en patient har type HLA A01/A02 og der foreligger blod fra to 5/6 match mulige navlesnore, hvor den ene unit har type HLA A01/A03 og den anden HLA A01/A01, vælges det navlesnorsblod med HLA A01/A01). Det samme forhold gælder også blod fra en 4/6 mismatched navlesnor.
