Marginal Zone Lymfom Senest revideret nov 2010 Marginal zone lymfomer (MZL) udgør en mindre, men heterogen gruppe af low-grade nonhodgkin B-celle lymfomer omfattende de primær extranodale slimhinde-associerede (MALT) lymfomer, primært kutane MZL, nodale MZL, og endelig de spleniske marginal zone lymfomer med eller uden villøse lymfocytter i blodet. Prognosen er varierende, men generelt god. Behandlingen er differentieret, og strækker sig lige fra helicobacter pylori erradikation og strålebehandling ved gastriske MALT lymfomer, over splenektomi ved splenisk MZL, til kombinationskemoterapi eller ”wait and watch”(Bertoni 2005, review). I sjældne tilfælde har sygdommen i avanceret stadie med udbredt nodal involvering et mere aggressivt forløb. Den optimale behandling heraf er ikke klarlagt. Autolog transplantation Der er ingen kontrollerede studier af højdosis terapi med stamcellestøtte ved marginal zone lymfomer. Der er kun enkelte retrospektive opgørelser i litteraturen. Autolog transplantation hos 11 patienter med avanceret, men kemofølsom sygdom, viste fortsat CR hos 5 af patienterne efter 52 måneder (Brown 2004). Patienterne konditioneredes med TBI og cyklofosfamid og stamcelleproduktet var purged med monoklonalt antistof. Overvejelse om autolog transplantation kommer dog kun sjældent på tale på grund af et oftest indolent forløb. Enkelte andre retrospektive opgørelser har beskrevet tilfælde af langvarig remission efter autolog transplantation hos patienter med transformeret MZL, omend prognosen er dårlig (Berger 2000; Neumeister 2000). In vivo purging med rituximab i forbindelse med stamcellemobilisering og høst kan medføre PCR-negative stamceller, hvilket predikterer opnåelse af molekylær remission efter transplantation og forlænget progressions fri overlevelse (Belhadj 2004). Rekommandation Eksperimentel. Autolog transplantion kan overvejes hos patienter under 65 år med tætte (< 6-12 måneder), udbredte og kemosensitive recidiver. Ved transformation til storcellet histologi er prognosen alvorlig, og højdosis terapi med autolog stamcellestøtte overvejes. Myeloablativ allogen HCT Der foreligger ingen systematiske opgørelser af allogen transplantation ved marginal zone lymfomer. I rapportering af resultater af allo-HCT ved low grade lymfomer vil patienter med MZL udgøre nogle få patienter ud af mange andre NHL subtyper, og man kan af den grund ikke udlede specifikke behandlingsresultater for marginal zone lymfomer. 75
