Objectives The use of MRI for the evaluation of osteonecrosis h<strong>as</strong> been established in recent<br />
years. As a persu<strong>as</strong>ive alternative to CT-scans, MRI is superior in the diagnosis of BRONJ.<br />
Nevertheless, a clear distinction of necrotic bone are<strong>as</strong> in c<strong>as</strong>es of BRONJ <strong>as</strong> well <strong>as</strong> a differentiation<br />
to surrounding soft tissues remains difficult. The purpose of this study w<strong>as</strong> to evaluate<br />
an enhancement of the illustration facilities in c<strong>as</strong>es of BRONJ by 3T-MRI.<br />
Material and methods: 15 patients with the diagnosis of BRONJ were examined by T1- and<br />
T2-weighted conventional and contr<strong>as</strong>t-enhanced 1.5T-MRI and 3D high resolution (0.8mm3<br />
isometric) MRI using a 3T-MRI unit (Siemens, Skyra, Erlangen, Germany). The evaluation of<br />
the MRI-scans w<strong>as</strong> performed by 2 maxillofacial surgeons, 1 radiologist and 1 neuroradiologist.<br />
All images were analysed independently by terms of preliminary criteria, followed by a comparison<br />
of the results.<br />
Results: Compared to 1.5T-MRI, high resolution 3T-MRI provided clearer images which allowed<br />
a better discrimination between necrotic are<strong>as</strong> and healthy bone in c<strong>as</strong>es of BRONJ. The visualization<br />
and reconstruction in a panoramic view simplifies the demonstration and illustration<br />
of intraosseous findings, especially in c<strong>as</strong>es of osteonecrosis.<br />
Conclusions: MR Imaging for visualization and detection of necrotic bone are<strong>as</strong> in patients<br />
with BRONJ remains the best opportunity, especially with the potential high-field 3T-MRI.<br />
Advantages in contr<strong>as</strong>t to conventional 1.5T-MRI are the better imaging distinction of the involved<br />
<strong>as</strong> well <strong>as</strong> a better differentiation to irritated surrounding soft tissues. More evaluations<br />
should follow to prove the potential benefit in c<strong>as</strong>es of BRONJ.<br />
