Body
Objectives: To develop a practical method for the evaluation of the success of secondary alveolar
bone grafting (SABG) in individuals with unilateral cleft lip and palate using CBCT.
Methods: A Promax 3D CBCT device (Planmeca, Helsinki) was used for the examination of
unilateral cleft lip and palate patients approximately 6 months after the SABG operation. The
interdental (i) height and (ii) width of the bone bridge after SABG, and (iii) the postoperative
height of the nasal floor in comparison to the unaffected side were examined. Kappa values
were calculated for inter- and intraobserver agreement.
Results: Interdental bone height was evaluated in the cervical, middle, and apical third of the
root. The bone height in these three portions was classified either to (i) no bone formation,
(ii) inadequate, or (iii) sufficient. The bone depth was classified similarly. The difference in
the height of the nasal floor in comparison to the unaffected side was measured in the coronal
slice in the apical level of the upper incisors.
Kappa values indicated excellent agreement (0.82-0.96), except for the fair agreement (0.53)
observed for the interexaminer width measurements in the middle third of the root. Because
of artefacts the width measurements in the cervical third of the root could be done only occasionally.
Kappa values for inter- and intraobserver agreement could not be calculated for the
latter measurements.
Conclusion: The evaluation of the success of SABG using CBCT with the presented method
proved to be reproducible and practical.
