Objectives: To report the treatment results for neck node met<strong>as</strong>t<strong>as</strong>is of oral cancer treated with<br />

surgical operation and postoperative radiotherapy (RT) at Osaka University Dental Hospital.<br />

Methods: Between 1986 and 2009, 75 patients (73 squamous cell carcinom<strong>as</strong> and 2 adenoid<br />

cystic carcinom<strong>as</strong>) with neck node met<strong>as</strong>t<strong>as</strong>is were treated with surgery and postoperative RT.<br />

Indications of the postoperative RT included <strong>as</strong> follows: single positive neck node with extra<br />

capsular spread of dise<strong>as</strong>e (18 patients, 24.0%, SWE group), multiple positive neck nodes<br />

without extra capsular spread of dise<strong>as</strong>e (23 patients, 30.7%, MWOE group) and multiple<br />

positive neck nodes with extra capsular spread of dise<strong>as</strong>es (26 patients, 34.7%, MWE group).<br />

The postoperative RT for all patients w<strong>as</strong> performed only neck area and irradiation dose ranged<br />

from 38 to 75 Gy (median, 54 Gy). The postoperative RT without chemotherapy w<strong>as</strong> performed<br />

in 47 patients and with chemotherapy w<strong>as</strong> performed in 28 patients.<br />

Results: The 5-year overall survival rate (OSR) for all patients w<strong>as</strong> 79.1%. The 5-year OSRs were<br />

88.8% for patients of SWE group, 91.7% for patients of MWOE group, and 69.2% for patients<br />

of MWE group. The 5-year OSRs were 80.5% for patients without chemotherapy and 76.5% for<br />

patients with chemotherapy. The 5-year OSRs were 60.0% for patients received less than<br />

50 Gy of postoperative RT and 85.2% for patients received equal or more than 50 Gy of postoperative<br />

RT.<br />

Conclusion: The postoperative RT seemed to be indispensable in some c<strong>as</strong>es after surgery for<br />

the neck node met<strong>as</strong>t<strong>as</strong>is in oral cancer patients.<br />
