IMMEDIATE-LOADING OF SMALL DIAMETER DENTAL IMPLANTS IN THE MANDIbLE<br />

Objectives: The mid-term evaluation of the success rate of small-diameter free-standing dental<br />

implants (2.4 mm diameter), the peri-implant tissue response, and complications (technical<br />

and surgical) when used to retain a mandibular overdenture in totally edentulous subjects.<br />

Materials and Methods: Ten completely edentulous patients (7 female, 3 males), were enrolled<br />

in this prospective study; average age 62.6 years (range 50.67 – 79.08 years). Each received<br />

new upper and lower complete dentures. Four sandbl<strong>as</strong>ted, acid-etched, self-tapping one-piece,<br />

screw-form small diameter implants (2.4 mm), of 13 mm length, were placed in the intraforaminal<br />

region of the mandible. Following insertion, all implants were immediately loaded with<br />

an overdenture. Implant stability and peri-implant tissues were examined clinically at 1 week,<br />

and at 1, 2, 3, 6, 12 and 24 months, and radiographically (panoramic radiograph) at b<strong>as</strong>e-line<br />

(day of surgery), 6 months post-insertion and at each 1-year interval after surgery. The patients<br />

were not allowed to remove the dentures themselves until the 3-month review. Statistical<br />

analysis w<strong>as</strong> adopted.<br />

Results: For the duration of this study, 24-months, the clinical success rate w<strong>as</strong> 100% (40<br />

implants in all). Clinical examinations were of modified Bleeding on Probing, modified Plaque<br />

Index, Gingival Index, Probing Depth. Each radiograph w<strong>as</strong> photographed and digitized and<br />

examined under a minimum magnification (x5). Each implant length, its individual magnification<br />

factor, The marginal crestal bone levels, mBOP, mPI, GI and PD showed no significant<br />

changes. No surgical or technical complications arose.<br />

Conclusion: Four small diameter implants used for mandibular overdenture retention are not<br />

inferior to large diameter implants<br />
