Objectives: To compare diagnostic validity of cone-beam computed tomography (CBCT) imaging<br />

with digital panoramic radiographs for orthodontic initial evaluation.<br />

Materials & Methods: From 20 subjects (age 6-30 years old, 10 males and 10 females), with<br />

a clinically justified need for both CBCT and panoramic images (taken with a maximum time<br />

interval of 6 months), images were collected (Oral Imaging Center, University hospitals KULeuven).<br />

Three observers scored image quality and criteria concerning orthodontic initial evaluation.<br />

The observation w<strong>as</strong> repeated after 4 weeks. The intra- and inter-observer agreement were<br />

evaluated.<br />

Results: CBCT images did not score significantly better than conventional digital panoramic<br />

radiographs concerning image quality. Yet, CBCT provided additional valuable diagnostic information<br />

for initial orthodontic treatment planning. The observer agreement w<strong>as</strong> moderate.<br />

Conclusions: While 2D-panoramic images are commonly used by orthodontists for orthodontic<br />

evaluation, CBCT images have the potential to provide additional valuable diagnostic information.<br />
