Objective:<br />
We report a rare c<strong>as</strong>e of pleomorphic adenoma (PA) of maxillary sinus with malignant transformation<br />
to squamous cell carcinoma with emph<strong>as</strong>is on radiographic features.<br />
Introduction:<br />
Malignant mixed tumors represent malignant counterparts to PA. These uncommon neopl<strong>as</strong>ms<br />
constitute 2 to 6% of all salivary gland tumors . The most common of these is carcinoma ex<br />
PA, which is characterized by malignant transformation of the epithelial component of a previously<br />
benign PA.Common sites of occurrence are the parotid and minor salivary glands of the<br />
palate.The malignant component is a poorly differentiated adenocarcinoma in most c<strong>as</strong>es but<br />
any type of carcinoma is probable.<br />
C<strong>as</strong>e report:<br />
A 50 year old woman referred to oral diagnostic department with a history of facial swelling<br />
in her left infraorbital area for one year. Clinical examination revealed exophthalmos of left<br />
eye. In panoramic radiography t<strong>here</strong> w<strong>as</strong> a destructive lesion causing complete opacification<br />
of left maxillary sinus. The posterior border of maxilla w<strong>as</strong> not visible. Computed tomography<br />
scans depicted a large ill- defined m<strong>as</strong>s in left maxillary sinus with destruction of medial,<br />
infraorbital and floor of the sinus extending to left orbital region. Histological diagnosis of<br />
the speciman obtained by incisional biopsy w<strong>as</strong> PA of minor salivary glands with malignant<br />
transformation in epithelial component to squamous cell carcinoma. The lesion w<strong>as</strong> completely<br />
excised and histopathologic examination confirmed the initial diagnosis.<br />
Conclusion:<br />
Malignant transformation of PA to squamous cell carcinoma is rare but it should be considered<br />
in differential diagnosis of destructive lesions of the maxillary sinus.<br />
