Body
Objective:
We report a rare case of pleomorphic adenoma (PA) of maxillary sinus with malignant transformation
to squamous cell carcinoma with emphasis on radiographic features.
Introduction:
Malignant mixed tumors represent malignant counterparts to PA. These uncommon neoplasms
constitute 2 to 6% of all salivary gland tumors . The most common of these is carcinoma ex
PA, which is characterized by malignant transformation of the epithelial component of a previously
benign PA.Common sites of occurrence are the parotid and minor salivary glands of the
palate. The malignant component is a poorly differentiated adenocarcinoma in most cases but
any type of carcinoma is probable.
Case report:
A 50 year old woman referred to oral diagnostic department with a history of facial swelling
in her left infraorbital area for one year. Clinical examination revealed exophthalmos of left
eye. In panoramic radiography there was a destructive lesion causing complete opacification
of left maxillary sinus. The posterior border of maxilla was not visible. Computed tomography
scans depicted a large ill- defined mass in left maxillary sinus with destruction of medial,
infraorbital and floor of the sinus extending to left orbital region. Histological diagnosis of
the speciman obtained by incisional biopsy was PA of minor salivary glands with malignant
transformation in epithelial component to squamous cell carcinoma. The lesion was completely
excised and histopathologic examination confirmed the initial diagnosis.
Conclusion:
Malignant transformation of PA to squamous cell carcinoma is rare but it should be considered
in differential diagnosis of destructive lesions of the maxillary sinus.
