Body
Objectives: To evaluate the influence of discrepancy between coronal articular shape on disk
shape and position, and thickness of roof of glenoid fossa (RGF).
Methods: This retrospective study included 45 patients who were diagnosed as unilateral internal
derangement of temporomandibular joint (TMJ) based on both magnetic resonance image
(MRI) and clinical examination (affected 45 joints, and 45 unaffected joints as control). The
coronal MR Images of condyles and fossae were divided into four groups according to modified
the Yale’s classification. Based on coronal shape of condyle and fassa in individual joints, all
joints were dichotomized into correspondence group (e.g. convex condyle and concave fossa)
and discrepancy (e.g. angled condyle and concave fossa) group. Evaluation of disk status and
measurement of RGF were performed on sagittal MRI. Statistical analyses were performed in
regard to follows: (1) discrepancy between condyle and fassa shape in the unaffected versus
affected side, (2) RGF thickness in the unaffected versus affected side, (3) disk status in correspondence
versus discrepancy groups in the affected side, and (4) RGF thickness in correspondence
versus discrepancy group in the affected side.
Results: Significant differences were found in above (1), (2) and (3); (1) odds ratio (OR) 5.69,
95% confidence interval (CI): 1.50-21.7, (2) unaffected side, 1.57 mm vs affected side, 1.81
mm (P=0.01), (3) OR 4.63, 95%CI: 1.08-19.7 in disk shape evaluation, no significance in disk
position evaluation.
Conclusion: These results suggested that discrepancy between condyle and fassa shape is a risk
factor of advanced internal derangement of TMJ.
