Objectives: To evaluate the influence of discrepancy between coronal articular shape on disk<br />
shape and position, and thickness of roof of glenoid fossa (RGF).<br />
Methods: This retrospective study included 45 patients who were diagnosed <strong>as</strong> unilateral internal<br />
derangement of temporomandibular joint (TMJ) b<strong>as</strong>ed on both magnetic resonance image<br />
(MRI) and clinical examination (affected 45 joints, and 45 unaffected joints <strong>as</strong> control). The<br />
coronal MR Images of condyles and fossae were divided into four groups according to modified<br />
the Yale’s cl<strong>as</strong>sification. B<strong>as</strong>ed on coronal shape of condyle and f<strong>as</strong>sa in individual joints, all<br />
joints were dichotomized into correspondence group (e.g. convex condyle and concave fossa)<br />
and discrepancy (e.g. angled condyle and concave fossa) group. Evaluation of disk status and<br />
me<strong>as</strong>urement of RGF were performed on sagittal MRI. Statistical analyses were performed in<br />
regard to follows: (1) discrepancy between condyle and f<strong>as</strong>sa shape in the unaffected versus<br />
affected side, (2) RGF thickness in the unaffected versus affected side, (3) disk status in correspondence<br />
versus discrepancy groups in the affected side, and (4) RGF thickness in correspondence<br />
versus discrepancy group in the affected side.<br />
Results: Significant differences were found in above (1), (2) and (3); (1) odds ratio (OR) 5.69,<br />
95% confidence interval (CI): 1.50-21.7, (2) unaffected side, 1.57 mm vs affected side, 1.81<br />
mm (P=0.01), (3) OR 4.63, 95%CI: 1.08-19.7 in disk shape evaluation, no significance in disk<br />
position evaluation.<br />
Conclusion: These results suggested that discrepancy between condyle and f<strong>as</strong>sa shape is a risk<br />
factor of advanced internal derangement of TMJ.<br />
