Aim – The aims of this project were to review the role of conventional ultr<strong>as</strong>ound and fine<br />

needle <strong>as</strong>piration cytology in the diagnosis of thyroid nodules and to undertake a small pilot<br />

clinical audit to <strong>as</strong>sess the benefits of ultr<strong>as</strong>ound el<strong>as</strong>tography.<br />

Method – In the pilot audit the data for the 14 patients who have in the p<strong>as</strong>t had a conventional<br />

ultr<strong>as</strong>ound, and real time ultr<strong>as</strong>ound el<strong>as</strong>tography examination using two different<br />

ultr<strong>as</strong>ound machines and fine needle <strong>as</strong>piration cytology were analysed. The results from<br />

conventional ultr<strong>as</strong>ound and real time ultr<strong>as</strong>ound el<strong>as</strong>tography images were compared with<br />

histopathological analysis.<br />

Results - In total, 17 nodules were examined. Conventional ultr<strong>as</strong>ound examination showed<br />

benign features for 11 nodules, indeterminate risk features for 5 nodules and abnormal<br />

suspicious features of malignancy for 1 nodule. All nodules which were found to be benign<br />

on ultr<strong>as</strong>ound el<strong>as</strong>tography were confirmed to be benign by fine needle <strong>as</strong>piration cytology.<br />

One nodule which showed malignant features on ultr<strong>as</strong>ound el<strong>as</strong>tography w<strong>as</strong> confirmed to be<br />

malignant on fine needle <strong>as</strong>piration cytology and histology.<br />

Conclusion – Indeterminate nodules presenting on conventional ultr<strong>as</strong>ound may not generally<br />

show any clear evidence that they are benign or malignant. Ultr<strong>as</strong>ound el<strong>as</strong>tography appears<br />

to offer an additional non-inv<strong>as</strong>ive tool to incre<strong>as</strong>e diagnostic confidence in this group.<br />

Perhaps in the future ultr<strong>as</strong>ound el<strong>as</strong>tography can be used to prevent unnecessary surgery by<br />

incre<strong>as</strong>ing diagnostic confidence, t<strong>here</strong>by reducing patient exposure to general anaesthetics<br />

and <strong>as</strong>sociated risks. It may also offer a more cost effective option to aid diagnosis.<br />
