OBJECTIVE:<br />

The aim of the investigation w<strong>as</strong> to <strong>as</strong>sess the influence of streak artefacts caused by amalgams<br />

during CBCT examination on detection of carious lesions located in the vicinity of<br />

amalgam fillings.<br />

MATERIAL AND METHOD:<br />

102 non-cavitated premolar and molar teeth were placed with approximal contacts consisting<br />

of three non-cavitated teeth and one MOD amalgam filled tooth in between. Radiographs of all<br />

the teeth were recorded using the CBCT system (NewTom 3G FOV9‘). Data from the CBCT unit<br />

were reconstructed and sectioned (0.25mm) in the mesiodistal tooth plane. Images were evaluated<br />

twice by two observers, using a five-step confidence scale. After the CBCT examination<br />

the teeth were individually sectioned in the mesiodistal direction with a diamond saw . Using<br />

a light microscope at 40 times magnification two different observers examined the tooth<br />

sections to established a true status of all approximal surfaces<br />

RESULTS:<br />

Sensitivity of the CBCT for the detection of caries on surfaces located proximally and distally<br />

to an amalgam filing w<strong>as</strong>: for enamel caries 0 and 0.07 respectively, for dentin caries 0.28<br />

and 0.29 respectively. Intraobserver reliability w<strong>as</strong> high and comparable for both observers<br />

(kappa coefficient 0.78 and 0.75 respecetively) but very poor interobserver agreement h<strong>as</strong><br />

been found (kappa coefficient 0.081) showing that the calculated values did not significantly<br />

differ from a random <strong>as</strong>signment.<br />

CONCLUSION:<br />

CBCT images are not suitable for caries diagnosis in c<strong>as</strong>es w<strong>here</strong> amalgam fillings are present in<br />

vicinity due to the extensive metal artefacts present.<br />
