Breath delivery<br />

• the set I:E RATIO is reached or inspiratory time has elapsed<br />

(PCV breaths).<br />

The ventilator delivers a VIM breath at the preset respiratory rate.<br />

The ventilator delivers a PIM breath if it detects a patient effort, as<br />

defined by the TRIGGER SENSITIVITY setting. The ventilator delivers<br />

an OIM breath if the operator presses MANUAL INSP.<br />

D.2 SPONT mode<br />

During SPONT mode, the ventilator delivers spontaneous breaths.<br />

Pressure support ventilation (PSV) is available in SPONT. In PSV the<br />

ventilator augments the patient’s inspiratory effort to sustain the set<br />

level <strong>of</strong> SUPPORT PRESSURE. On the 760 Ventilator only, the RISE TIME<br />

FACTOR setting determines how quickly the target pressure is<br />

achieved, and the EXH SENSITIVITY setting determines the point at<br />

which the breath cycles to exhalation. When the inspiratory phase <strong>of</strong> a<br />

PSV breath terminates, the exhalation valve opens and pressure falls<br />

to the set level <strong>of</strong> PEEP/CPAP. The patient triggers all spontaneous<br />

breaths, and influences their inspiratory flow, inspiratory time, and<br />

tidal volume.<br />

The ventilator normally terminates the inspiratory phase <strong>of</strong> a PSV<br />

breath when:<br />

• inspiratory flow drops to 10 L/min or 25% <strong>of</strong> peak flow, whichever<br />

is less (740 Ventilator), or when the inspiratory flow is less than the<br />

set value <strong>of</strong> EXH SENSITIVITY (760 Ventilator), or<br />

• patient wye pressure exceeds the set inspiratory pressure<br />

(Pi + PEEP) by more than 3 cmH2O (3 hPa), or<br />

• inspiratory time exceeds 3.5 seconds (2.5 seconds for pediatric<br />

ventilator breathing circuits).<br />

The ventilator delivers an OIM breath during SPONT when an operator<br />

presses MANUAL INSP. OIM breaths delivered during SPONT are based<br />

on the apnea settings.<br />
