5.13.2.4. One pilot should record and read back all ATC clearances; The navigator or<br />

communications systems operator (if applicable) will record the clearance instructions,<br />

monitor the read back, and ensure compliance with all instructions.<br />

5.13.2.5. Both pilots will monitor UHF and VHF guard emergency frequencies to the<br />

maximum extent possible. To monitor VHF guard, 121.5 must be tuned.<br />

5.13.2.6. The Federal Communications Commission (FCC) prohibits the use of<br />

unauthorized frequencies for interplane, HAVE QUICK, or SECURE VOICE training.<br />

5.14. Crew Resource Management (CRM)/Threat and Error Management.<br />

5.14.1. Threat and Error Management provides strategies and tactics to help crews target<br />

threats to safe flight operations and decreases the potential for crew error. External threats<br />

are events that occur outside the influence of the flight crew and require crew attention and<br />

management to maintain adequate safety margins. Internal threats are crew related and are<br />

factors that could lead to an error if not recognized and controlled.<br />

5.14.2. "Time Out" is the common assertive statement for use by all crewmembers. The use<br />

of "Time Out" will:<br />

5.14.2.1. Provide a clear warning sign of a deviation or loss of situational awareness.<br />

5.14.2.2. Provide an opportunity to break the error chain before a mishap occurs.<br />

5.14.2.3. Notify all crewmembers when someone sees the aircraft or crew departing from<br />

established guidelines, the briefed scenario, or that someone is simply uncomfortable<br />

with the developing conditions.<br />

5.14.2.4. As soon as possible after a "Time Out" has been called, the aircrew will take<br />

the following actions:<br />

5.14.2.5. Safety permitting, stabilize the aircraft and ensure terrain clearance.<br />

5.14.2.6. The initiating crewmember will voice their concerns to the crew.<br />

5.14.2.7. The PIC will provide all other crewmembers with the opportunity to voice<br />

inputs relative to the stated concerns.<br />

5.14.2.8. After considering all inputs, the PIC will direct the aircrew to continue the<br />

current course of action or direct a new course of action.<br />

5.14.3. Sterile Cockpit. With the exception of cruise flight, conversation below 18,000‘<br />

MSL will be limited to mission, departure, or approach essential items. Every effort will be<br />

made to accomplish briefings and appropriate checklists prior to top of descent (TOD).<br />

Sterile cockpit procedures also apply during taxi, low level, and air refueling operations.<br />

5.14.4. Heads-up/Heads-down. Any crewmember that observes both pilots heads-down at<br />

the same time (other than heads-down instrument flying) shall alert the PF without delay.<br />

5.14.5. CRM Enhancement. PICs should conduct a CRM exercise on the first suitable<br />

segment of each mission. This will be done at level off on a non-interference basis with<br />

other mission requirements. Take the exercise to a logical conclusion and ensure crew<br />

communications and duties are appropriate. Suggested topics are rapid decompression,
