consciousness-based education and physiology and healthA more recent review <strong>of</strong> 58 studies found reductions in use <strong>of</strong> medicalservices associated with inpatient rather than outpatient mental healthcare, particularly for persons over 55 years <strong>of</strong> age (3). Twenty-two randomassignment studies reviewed indicated a 10.4% mean reduction ininpatient hospitalization compared with 33.1% mean reduction in 26time series studies in which patients had self-selected psychotherapytreatment. Both changes were statistically significant, indicating thatself-selection studies yield reliable information, albeit a larger effect size.A meta-analysis <strong>of</strong> 49 experiments on the effects <strong>of</strong> psycho educationalinterventions with surgical patients indicated that method <strong>of</strong> subjectassignment was not systematically related to effect size, i.e., the outcomeswere similar for random assignment and self selection studies (4).The present study analyzes five years <strong>of</strong> medical care utilization statisticsfrom a major health insurance carrier on a self-selected group<strong>of</strong> approximately 2000 participants in the Transcendental Meditation(TM) program <strong>of</strong> <strong>Maharishi</strong> Mahesh Yogi (5). There are abouttwo million Transcendental Meditation meditators worldwide and theTranscendental Meditation technique is not a therapeutic modality perse but is <strong>of</strong>fered to the general public as a means <strong>of</strong> self-actualization(5). It is taught in seven systematic steps, which include introductoryand preparatory lectures, personal interview and personal instruction,and verification and validation <strong>of</strong> experiences <strong>of</strong> the first three days<strong>of</strong> meditation (6, p. 6). The technique is taught by qualified teacherstrained by <strong>Maharishi</strong> and it is described as “an effortless procedure forallowing the excitations <strong>of</strong> the mind gradually to settle down until aleast excited state <strong>of</strong> mind is reached” (6, p. 123).The Transcendental Meditation program consists <strong>of</strong> the regularpractice <strong>of</strong> the Transcendental Meditation technique 15–20 minutestwice a day. It is practiced at home, in the <strong>of</strong>fice, or wherever is convenient.It does not require any special treatment setting, and although aquiet place is best, it can also be practiced successfully in noisy environmentssuch as a bus, commuter train, or airplane.Physiologic research shows that the Transcendental Meditationtechnique produces a state <strong>of</strong> “restful alertness” (7, 8). Rest is indicatedby reduced minute ventilation, respiration rate, plasma lactate levels,and spontaneous skin resistance responses and increased basal skinresistance relative to eyes-closed rest in non-meditating control subjects324
