Flying Deck Form

9.0 See Section 2, "General Requirements for Shoring".

9.1 Flying deck forms shal! have allowable loads as determined by detailed structural analysis. The results of the

structural analysis shall be verified by tests conducted under, or equal to, field conditions, witnessed by an

independent authority.

9.2 Design of shoring layouts using flying deck form systems shall be based on the above results and using factors

of safety as follows: Trusses and proprietary or component supporting members - 2.5 to 1; all other members

as per this shoring standard.

9.3 All flying deck components shall be inspected before use to insure that the erected form conforms to the shoring

layout, that correct components are being used, and that no damaged components are being used.

9.4 Follow manufacturers' instructions, load ratings, and job site layout drawings for placement on each project.

9.5 No components shall be used if they are heavily rusted, bent, dented, torn, or otherwise defective.

9.6 All components are to be in good working order and in a condition similar to that of original manufacture.

9.7 All screw jacks and connections shall be checked for snugness.

9.8 Special precautions shall be taken by the user when the flying deck form is angling, sloping, or cantilevered.

9.9 Adjustment of flying deck forms to raise formwork shall not be made during concrete placement.

9.10 A method of positive control must be provided for the rolling-out operation by tie-back, braking system or other

devices. The form must never be allowed to have uncontrolled horizontal movement.

9.11 Positive connection between the form and the crane must be provided by slings, eyes, or other devices

recommended by the manufacturer.

9.12 Follow manufacturers' recommendation for roll-out, flying, and landing of the flying deck form.
