SECTION 9 — FLYING DECK FORMS

9.13 A tag line must be attached to the flying deck form to control movement while flying, and for helping to position

the form when landing.

9.14 Flying of deck forms during high winds is hazardous and not recommended.

9.15 Crane pick points shall not be changed without consulting with the form manufacturer.

9.16 Worker shall not be permitted to be on a flying deck form when it is completely attached to a crane and is being

moved to its new location.

9.17 When shoring high slabs that require the use of long extension legs, diagonal bracing in both lateral planes

resulting in braced leg groups of four (4) must be installed in accordance with manufacturer's recommendations.

9.18 Filler and hinged panels result in concentrated loads on the edge of the flying deck system and must be taken

into consideration during movement and flying.

9.19 It is unsafe and unlawful to allow employees on flying deck forms without proper guardrailing. If guardrailing is

impractical, employees must be properly protected by other equivalent means, such as safety belts and lanyards.

9.20 Use extreme caution against tipping hazards; small, light tables may be susceptible to tipping by weights of

personnel and materials. Such tables must be restrained from tipping by rear tie-downs or front support shores

and no personnel must be allowed on the deck until such restraints are installed.

9.21 Horizontal shoring beams shall be spaced over and clipped to the top chord of the flying form in accordance with

supplier's drawings.

9.22 Plywood decks on flying forms shall have:

(1) Face grain running at right angles to supporting horizontal shoring beams.

(2) Conform to A.PA Plyform Class 1, B-B Exterior Grade in accordance with PSI-66 in "as new"

condition, or be of equal or greater strength.

(3) Edge-Nailed to the nailer strip on the horizontal shoring beam at supplier's recommended

spacing.
