Admissions<br />

Policy<br />

Admission as aregular student to <strong>Cochise</strong> <strong>College</strong> may be<br />

granted to any person who meets one of the following criteria:<br />

1. Is agraduate of a high school which is accredited by a<br />

regional accrediting association.<br />

2. Has a high school certificate cf equivalency, such as aGED.<br />

3. Is 18 years of age or older on or before the first day of classes<br />

for which applicatiorl is being made.<br />

4. Is aregular transfer student in good standing from another<br />

college or university.<br />

Curr~nt high school students or others under age 18 may be<br />

admitted through aspecial policy. Contact the Admissions Office<br />

for further information.<br />

The Admissions Office invites anyone interested in <strong>Cochise</strong><br />

<strong>College</strong> to visit the Douglas or Sierra Vista Campus to learn more<br />

about programs of study and available services. The Admissions<br />

Office is open Monday through Friday, 8a.m. to 4:30 p.m.<br />

Campu~ tours ~re available throughout the year. Call or write the<br />

Admissions Office for an appOintment.<br />

Procedure<br />

Applicants are admitted to <strong>Cochise</strong> <strong>College</strong> after the Office of<br />

Admissions has received and approved:<br />

• Application for Admission.<br />

• $5 application fee for out·of·state students.<br />

• Student Medical Record,; Part I. - ThiS self-appraisal form is<br />

to ~e,completed, by all applicants who are applying for the<br />

AViation or NurSing programs or who wish to live in the residence<br />

halls, at the DO,uglas Ca~pus, The <strong>College</strong> reserves the right to<br />

require a phYSical examln~tion when deemed necessary by the<br />

<strong>College</strong> Nurse or by aparticular college instructional program,<br />

• High School Transcriptl- AU applicants under the age of 16,<br />

who are not COfICUrrenUy enrolled in an accredited high school,<br />

must supply either aofficial high school transcript or GED<br />

certification with test scores before registration will be permitted,<br />

Final high school transcripts or GED certificate may be required of<br />

all students who apply for any Financial Aid Program offered<br />

through <strong>Cochise</strong> <strong>College</strong>. High school transcripts mu~ ; sent<br />

directly by the sending institution to the Admissions Office.<br />

Transcripts carried by the applicant will not be accepted.<br />

•<strong>College</strong> Transcripts - Applicants who have attended college<br />

elsewhere must submit an official transcript of college work in<br />

order for acourse or courses to be considered by <strong>Cochise</strong><br />

<strong>College</strong> for gradu&:ion purposes. <strong>College</strong> transcripts are to be<br />

sent directly by the sending institution to the Transcript Office,<br />

Transcripts carried by the applicant will not be accepted.<br />

• Report of Physical Examination; Part II. - Persons planning<br />

to participate in intercollegiate athletics and those who are<br />

residents of foreign countries must have this form completed by a<br />

licens,ed physician. ~ursjng applicants must complete aphysical<br />

examination determined by the <strong>College</strong> Nursing Program.<br />

• Out of County Affidavits. - The affidavit is required for all<br />

residents of Apache, Gila, Greenlee and Santa Cruz counties<br />

before registration each semester,<br />

<strong>College</strong> entrance examinations, such as ACT or SAT are not<br />

required for ,admission. However, the <strong>College</strong> does administer the<br />

A,CT on testing dates throughout the year as a service to County<br />

high school students and residents who wish to take the test.<br />

Students with Special Needs<br />

If yo~ are a person with adisability and require any auxiliary aids.<br />

services or other accommodations as astudent of <strong>Cochise</strong><br />

<strong>College</strong>, please in,dicate same on your admissions application<br />

and arepresentative of the college will contact you to discuss<br />

your accommodation needs,<br />

Di~closur~ of Social Security Account Number<br />

SOCial security acc,ount numbers will be used solely to identify<br />

student records, Disclosure and use of social security account<br />
