ANNEX F (SUSTAINMENT) TO OPERATION PLAN/ORDER [number] [(code name)]—[issuing<br />

(1) (U) Medical Evacuation. Provide medical evacuation information. Address the theater<br />

evacuation policy, en route care, medical regulating (if appropriate), casualty evacuation, <strong>and</strong> the medical<br />

evacuation of casualties contaminated with chemical, biological, radiological, <strong>and</strong> nuclear ordnance.<br />

(2) (U) Hospitalization. Provide hospitalization information <strong>and</strong> guidelines. List the locations of<br />

medical treatment facilities. Identify <strong>and</strong> list area units without organic medical resources requiring support<br />

<strong>and</strong> describe how to support these units. Describe the procedures for mass casualty operations <strong>and</strong> patient<br />

decontamination operations. Identify <strong>and</strong> list levels of medical care (I, II, III, <strong>and</strong> IV) by treatment facility<br />

<strong>and</strong> location. Refer to Tab A (Sustainment Overlay) to Appendix 1 (Logistics) to Annex F (Sustainment) <strong>and</strong><br />

Appendix 3 (<strong>Army</strong> Health System Support) to Annex F (Sustainment) as required.<br />

d. (U) Foreign Nation <strong>and</strong> Host-Nation Support. Provide host-nation support information. Refer to<br />

Annex P (Host-Nation Support) as required.<br />

e. (U) Resource Availability. Identify significant competing dem<strong>and</strong>s for sustainment resources where<br />

expected requirements may exceed resources.<br />

f. (U) Miscellaneous. Provide any general miscellaneous information not covered in this annex.<br />

5. (U) Comm<strong>and</strong> <strong>and</strong> Signal.<br />

a. (U) Comm<strong>and</strong>.<br />

(1) (U) Location of <strong>Comm<strong>and</strong>er</strong>. State the location of sustainment area leaders.<br />

(2) (U) Succession of Comm<strong>and</strong>. State the succession of comm<strong>and</strong> if not covered in the unit’s<br />

SOPs.<br />

(3) (U) Liaison Requirements. State the sustainment liaison requirements not covered in the base<br />

order.<br />

b. (U) Control.<br />

(1) (U) Comm<strong>and</strong> Posts. Describe the employment of sustainment-specific comm<strong>and</strong> posts (CPs),<br />

including the location of each CP <strong>and</strong> its time of opening <strong>and</strong> closing.<br />

(2) (U) Reports. List sustainment-specific reports not covered in st<strong>and</strong>ard operating procedures.<br />

Refer to Annex R (Reports) as required.<br />

c. (U) Signal. Address any sustainment-specific communications requirements. Refer to Annex H<br />

(Signal) as required.<br />
