It classicallypresents on only one side <strong>of</strong> <strong>the</strong> cortex <strong>of</strong> long bones. It is common in synovial joints andcrosses <strong>the</strong> joint line in 35% cases. Its etiology is unknown and it is a benign condition but <strong>the</strong>reare 3 reported cases in literature associated with malignancy. Chronic pain and deformity frommelorheostosis can be severely debilitating for patients. Its management in most cases isconservative. Surgical intervention is advocated in chronic debilitating symptoms. Successfulresection <strong>of</strong> <strong>the</strong>se lesions can translate into near complete resolution <strong>of</strong> <strong>the</strong> symptoms. Thirty-five (0.67%) peri-pros<strong>the</strong>tic fractures that weretreated operatively in 32 patients were identified and classified using <strong>the</strong> VancouverClassification. There were 9 patients with a type Agfracture, 2 patients with a type B1 fracture, 17 patients with a type B2 fracture, 1 patient with atype B3fracture, and 3 patients with a concomitant type Ag and B2 fracture. Eleven (34%) patientswere treatedwith isolated ORIF: greater trochanter (9) or femoral shaft (2).
