The purpose <strong>of</strong> this study is to evaluate <strong>the</strong> thickness <strong>of</strong> <strong>the</strong> cementmantle and grafted layer, and cement penetration to <strong>the</strong> grafted layer in <strong>the</strong> improved IBGtechnique by experimental model.Materials and methods: Frozen swine femoral head and distal femur were morsellized by combtype bone mill. Mosellized bone was impacted into five sawbones (model no. 1193, Sawbones,USA). YU revision system (JMM, Osaka, Japan) was used for IBG procedure. The canal <strong>of</strong>femur was completely packed with morsellized bone at first and reamed by drill through <strong>the</strong>guide pin. Finally, <strong>the</strong> medullar space was finished up with <strong>the</strong> stem impactor and bone pusher.Helical CT scan (Aquilion 16, Toshiba, Japan) was examined after removal <strong>of</strong> <strong>the</strong> stem toanalyze <strong>the</strong> thickness <strong>of</strong> <strong>the</strong> cement mantle and grafted layer, and cement penetration to <strong>the</strong>grafted layer.Results: Average thickness <strong>of</strong> cement mantle was 1.32 mm (0.84 - 2.02 mm) and averagethickness <strong>of</strong> Grafted layer was 3.26 mm (1.08 - 10.07 mm). Cement penetration was found atany slice level, and some <strong>of</strong> which reached to <strong>the</strong> endosteal <strong>of</strong> <strong>the</strong> sawbone.Discussion: IBG technique with morselized allogenic bone to <strong>the</strong> patient with small femur andnarrow canal is difficult. Grafted layer tends to insufficient especially at middle part <strong>of</strong> <strong>the</strong>implant. Our modified IBG technique is useful to make <strong>the</strong> grafted layer safety and uniformly.It is reported <strong>the</strong> structure characteristic <strong>of</strong> <strong>the</strong> cement and grafted morsellized bone complex isone <strong>of</strong> <strong>the</strong> important factor on primary stability <strong>of</strong> <strong>the</strong> implant.
