Whereas severe pre-operative varus deformity needs substantial soft tissuerelease on the medial side during TKA, what extent to be released along the medial structures isstill controversial. Using a tensor for TKA that is designed to facilitate soft tissue balancemeasurements throughout the range of motion with a reduced patello-femoral (PF) joint andfemoral component in place, we examined the influence of pre-operative deformity on intraoperativesoft tissue balance during posterior-stabilized (PS) TKA. [Methods] From a group of60 consecutive osteoarthritic females performed the same type of implant received (NexGenLPS Flex, Zimmer, Inc,. Warsaw, IN), we retrospectively divided into three groups dependingon the pre-operative coronal plane alignment; 20 patients with less than 10 degrees of preoperativevarus alignment, 20 patients with more than 10 degrees and less than 20 degrees ofpre-operative varus alignment, and 20 patients with more than 20 degrees of pre-operative varusalignment. After excluding patients with valgus deformity and severe bony defects predictingthe possibility of the use of augmentation block or bone graft, each remaining patient had avarus deformity, with an average pre-operative coronal plane alignment of 4.6 ± 0.6º in thevarus alignment < 10º group, 14.2 ± 0.3º in the 10º < varus alignment < 20º group, and 23.3 ±1.0º in the varus alignment > 20º group. Surgeries were performed with measured resectiontechnique by a single senior surgeon.
