Whereas severe pre-operative varus deformity needs substantial s<strong>of</strong>t tissuerelease on <strong>the</strong> medial side during TKA, what extent to be released along <strong>the</strong> medial structures isstill controversial. Using a tensor for TKA that is designed to facilitate s<strong>of</strong>t tissue balancemeasurements throughout <strong>the</strong> range <strong>of</strong> motion with a reduced patello-femoral (PF) joint andfemoral component in place, we examined <strong>the</strong> influence <strong>of</strong> pre-operative deformity on intraoperatives<strong>of</strong>t tissue balance during posterior-stabilized (PS) TKA. [Methods] From a group <strong>of</strong>60 consecutive osteoarthritic females performed <strong>the</strong> same type <strong>of</strong> implant received (NexGenLPS Flex, Zimmer, Inc,. Warsaw, IN), we retrospectively divided into three groups dependingon <strong>the</strong> pre-operative coronal plane alignment; 20 patients with less than 10 degrees <strong>of</strong> preoperativevarus alignment, 20 patients with more than 10 degrees and less than 20 degrees <strong>of</strong>pre-operative varus alignment, and 20 patients with more than 20 degrees <strong>of</strong> pre-operative varusalignment. After excluding patients with valgus deformity and severe bony defects predicting<strong>the</strong> possibility <strong>of</strong> <strong>the</strong> use <strong>of</strong> augmentation block or bone graft, each remaining patient had avarus deformity, with an average pre-operative coronal plane alignment <strong>of</strong> 4.6 ± 0.6º in <strong>the</strong>varus alignment < 10º group, 14.2 ± 0.3º in <strong>the</strong> 10º < varus alignment < 20º group, and 23.3 ±1.0º in <strong>the</strong> varus alignment > 20º group. Surgeries were performed with measured resectiontechnique by a single senior surgeon. Following each osteotomy, we removed osteophytes,released <strong>the</strong> posterior capsule along <strong>the</strong> femur, and corrected any ligament imbalances thatoccurred in <strong>the</strong> coronal plane by releasing s<strong>of</strong>t tissues along <strong>the</strong> medial structures <strong>of</strong> <strong>the</strong> kneeaccording to <strong>the</strong> following criteria; (1) more than 20 mm <strong>of</strong> medial gap between <strong>the</strong> cuttingfile:///E|/<strong>ISTA</strong>2010-Abstracts.htm[12/7/2011 3:15:47 PM]
