OBJECTIVE. Develop a technique to restore <strong>the</strong> articular surface with bone graft impactedthrough surgical hip dislocation approach in patients with AVN <strong>of</strong> <strong>the</strong> femoral head Ficat 4 toprevent early articular degeneration.MATERIAL AND METHODS. We performed a prospective study in 5 patients with AVN <strong>of</strong>femoral head Ficat 4, 32 years old average (24-39). We treated all <strong>of</strong> <strong>the</strong>m with a reshape <strong>of</strong><strong>the</strong> femoral head with bone graft through surgical hip dislocation approach, all got a completespherical head after <strong>the</strong> procedure. They were follwed up with radiographs , CT and Harris HipScore (HHS) at 3, 6, 12 months and <strong>the</strong>n annually.RESULTS. We obtained a complete spherical head in all cases, and restitution <strong>of</strong> <strong>the</strong> articularsurface after surgery. HHS initially showed an improvement during <strong>the</strong> first six months (20points average) Never<strong>the</strong>less patients began with progressive deterioration ei<strong>the</strong>r insymptomatology and imaging. At 24 months after surgery (18-36) all patients had returned topreoperative HHS, radiographs and CT showed a complete collapse in <strong>the</strong> bone grafted zone.All patients needed a total hip replacement at that moment. There were no o<strong>the</strong>r associatedcomplications.CONCLUSIONS. The bone graft for <strong>the</strong> treatment <strong>of</strong> AVN <strong>of</strong> <strong>the</strong> femoral head Ficat 4, hasno impact on <strong>the</strong> natural history <strong>of</strong> <strong>the</strong> disease. Patients had no clinical nor imagingimprovement at short term follow up.
