The limitation <strong>of</strong> this meta-analysis was that out <strong>of</strong> <strong>the</strong> 6included studies, only one was methodologically well designed and reported.The findings from our baseline audit also support <strong>the</strong> findings from this meta-analysis, howeverwe need to be careful in our interpretation <strong>of</strong> <strong>the</strong>se significant findings. It is well known thatcertain patient factors like smoking, obesity, diabetes and steroid use increase incidence <strong>of</strong>infections. Of our 5 patients with infected wounds one had a preceding chest infection, onesuffered from recurrent UTI’s and ano<strong>the</strong>r one was an immunosuppressed renal patient. Thusstaples cannot be blamed for all superficial wound infections.Our aim now will be to present <strong>the</strong>se findings and to ei<strong>the</strong>r initiate a well designed randomisedcontrol trial to answer this debate between staples and sutures, or to accept <strong>the</strong> findings andchange our practice by closing all skin wounds with sutures and re-audit in 6 months time.Poster: 92Modified Fulkersons Procedure, Combined With Micr<strong>of</strong>racture for Patello-Femoral Arthritis in <strong>the</strong> Younger Patient*Ahsan Sheeraz - Royal National Orthopaedic Hospital, Stanmore - London, UKPaul Allen - Princess Royal University Hospital - Orpington, UK*Email: ahsanshiraz@hotmail.comWe report <strong>the</strong> results <strong>of</strong> a modified Fulkerson technique <strong>of</strong> antero-medialisation <strong>of</strong> <strong>the</strong> tibialtubercle, combined with micr<strong>of</strong>racture or abrasion arthroplasty in patients <strong>under</strong> 60 withpatello-femoral osteoarthritis.All patients operated on between September 1992 and October 2007 were reviewed by anindependent observer in clinic or by postal questionnaire, using <strong>the</strong> Oxford Knee Score,Melbourne Patella Score and a Satisfaction Score. Only patients with Outerbridge Grade 3 – 4osteoarthritis <strong>of</strong> <strong>the</strong> patello-femoral joint were included. They were assessed pre-operativelywith plain x-rays, MRI scans (as well as tracking scans in <strong>the</strong> last 10 years) andarthroscopically. All patients with tracking scans showed lateral subluxation <strong>of</strong> <strong>the</strong> patella.The surgical procedure was a modification <strong>of</strong> Fulkerson’s tibial tubercle osteotomy, with anadvancement <strong>of</strong> 1-1.5 cms and a medialisation <strong>of</strong> 1.5 cms.
