Poster: 83Defect Management in Total Knee Arthroplasty*Ashok Rajgopal - Fortis Hospital,New Delhi - New Delhi, India*Email: arajgopal@hotmail.comWith <strong>the</strong> increase in <strong>the</strong> numbers <strong>of</strong> revision total knee arthroplasty (TKA) we encounterincreased instances <strong>of</strong> bone defects. Defects are classified based on <strong>the</strong>ir location, whe<strong>the</strong>rcontained or not, and <strong>the</strong>ir size. The principles <strong>of</strong> managing defects in <strong>the</strong> femur and tibial aresimilar and are described toge<strong>the</strong>r. Contained defects can be dealt with by using bone cement orbone graft. If <strong>the</strong> defect involves more than 1/3 <strong>of</strong> <strong>the</strong> tibial plateau <strong>the</strong>n a tibial stem isinserted. Large defects with one column intact are amenable to reconstruction by use <strong>of</strong>impaction bone grafting or trabecular metal augments. For massive defects involving both <strong>the</strong>columns <strong>the</strong> choice is between trabecular metal augments and allografts. An intra-medullarystem, mesh or locking plates can be used to achieve stability. Bony defects in <strong>the</strong> patella aredifficult to manage. If <strong>the</strong> implant is well fixed at <strong>the</strong> time <strong>of</strong> revision <strong>the</strong>n it is best left alone.Patellar defects if contained can be bone grafted, if not contained <strong>the</strong>n a smaller patellar implantcan be used.
