In this study, weperformed randomized controlled trial to examine whe<strong>the</strong>r intraarticularinjection <strong>of</strong> TXA decreases leg swelling by reducing <strong>the</strong> blood loss after TKA.Materials and Methods: We performed one-hundred TKA with <strong>the</strong> patients <strong>of</strong> OA(men; 12 women; 88). At <strong>the</strong> closure, total <strong>of</strong> 2000 mg/20 mL TXA was injectedinto <strong>the</strong> knee joint through closed suction drain (TXA group). For <strong>the</strong> controlgroup, same volume <strong>of</strong> physiological saline was injected (control group). Theclosed drain was gradually opened post 30 minutes after injection.Preoperative conditions <strong>of</strong> <strong>the</strong>se patients were examined and post operativehaemoglobin (Hb) level, blood loss through drain, D-dimer, and needs fortransfusion were compared with <strong>the</strong>se two groups. Fur<strong>the</strong>rmore, circumferentialmeasurements <strong>of</strong> <strong>the</strong> leg (thigh, suprapatellar portion, and calf girth) weremeasured at pre, and post operation to investigate whe<strong>the</strong>r TXA influences <strong>the</strong>leg swelling post operation.Results: Postoperative decrease in Hb level was significantly down-regulatedin TXA group. Blood loss through drain were suppressed by 6 hours postoperation in TXA group, however <strong>the</strong> total blood loss through drain was <strong>the</strong>same among <strong>the</strong>se groups. Fur<strong>the</strong>rmore, supra-patellar girth at 1 week and calf girth at 2weeks post operation were significantly thinner in TXA group than in controlgroup.Conclusions: The results in this study indicated TXA decreases leg swelling via reducingblood loss.
