One major factor affecting the wear behaviour of the PEinsert in-vivo is the surface characteristics of the articulating femoral components.Contemporary femoral components available in Canada are either made of cast CobaltChromium (CoCr) alloy or have an oxidized zirconium surface (Oxinium). The latter type offemoral components have shown to have increased abrasive wear resistance and increasedsurface wettability, thus leading to reduced PE wear in-vitro compared with conventional castCoCr components. Although surface damage has been reported on femoral components ingeneral, there have been no reports in the literature as to what extent the recommendedoperating techniques affect the surface tribology of either type of femoral component.Twenty-two retrieved total knee replacements were identified with profound surface damage onthe posterior aspect of the femoral condyles. The femoral components were of three differentknee systems: five retrievals from the NexGen® total knee system (Zimmer Inc., Warsaw, IN),twelve retrievals from the Genesis II® total knee system (CoCr alloy or Oxinium; Smith &Nephew Inc., Memphis, TN), and five retrievals from the Duracon® total knee system (StrykerInc., Mahwah, NJ). Reasons for revision were all non-wear-related and included asepticloosening in two cases, painful flexion instability, and chronic infection. All retrieved femoralcomponents showed evidence of surface damage on the condyles, at an average of 99º flexion(range, 43º – 135º flexion). Titanium (Ti) alloy transfer and abrasive surface damage wereevident on all retrieved CoCr alloy femoral components that came in contact with Ti alloy tibialtrays. Surface damage on the retrieved Oxinium femoral components was gouging, associatedwith the removal and cracking of the oxide and exposure of the zirconium alloy substratematerial. CoCr alloy femoral components that had unintended contact with CoCr alloy tibialtrays also showed evidence of gouging and abrasive wear.All femoral components showed severe surface damage in the posterior aspect of the condyles.The femoral surface was heavily scratched and the oxidized zirconium coating surface appearedremoved. The surface analysis suggested that the surface damage most likely occurred duringthe time of initial implantation. In particular, it appeared that the femoral condyles were restingon the posterior aspect of the tibial tray in flexion, thus scratching the femoral components.Such scratches could potentially lead to accelerated PE insert wear and reduced implantlongevity, thus making expensive revisions surgery necessary. The use of this system is diverse and now applicable forVertebroplasty, Biopsy procedures and different techniques of Spinal fusion. During this timeour clinic performed near 150 robotic assisted surgeries.Amongst its benefits the system allows the reduction of the duration of fluoroscopic exposure inthe OR, better accuracy due to computerized assisted planning and navigation, avoidance ofhuman caused complications and a less traumatic procedure for the patient. On the other hand,the duration of the procedure is prolonged, the wound is subdued to a longer exposure in casesof the open surgery, and the operational cost is higher and requires a good trained medical staff.Materials and Methods: In the last 2 years we have performed 56 robotic assistedVertebroplasty procedures (research group).
