Disfunción renal (nefritis túbulo intersticial) con baja<br />
incidencia. Evaluar función renal antes de prescribirlo y<br />
ajustar dosis si la DepCreat menor a 50 ml/min. Evitar<br />
otros nefrotóxicos. Atazanavir y lopinavir/r aumentan<br />
niveles séricos de TDF (vigilar toxicidad, posible mayor<br />
riesgo de daño renal). Intolerancia GI. Reducir dosis de<br />
ddI. Exacerbación de hepatitis por virus B, al suspenderlo.<br />
Inhibidores no nucleósidos de la transcriptasa reversa (INNTR)<br />

Comentario<br />
Efectos diversos en SNC (frecuentes pero pasajeros):<br />
mareos, insomnio, somnolencia, sueños vívidos,<br />
confusión agitación, depresión aguda; se resuelven en 2-4<br />
semanas.<br />
Rash (10%; grave en menos del 2%).<br />
Teratogénico: evitarlo en embarazadas y en mujeres con<br />
potencial reproductivo.<br />
Elevación de ALT/AST. La coinfección con hepatitis B o<br />
C incrementa riesgo de toxicidad hepática. Dislipidemia.<br />
Rash: 7% grave (Stevens-Johnson), potencialmente fatal<br />
en 2%.<br />
Hepatitis aguda (a veces fatal). En mayor riesgo:<br />
coinfección con hepatitis B o C, mujeres con CD4 mayor<br />
a 250 células/mm 3 ; embarazadas; hombres con CD4<br />
mayor a 400 células/mm 3 . Contraindicado en<br />
enfermedad hepática Child-Pugh B o C.<br />
