6.4 - Scleroterapia con trans-illum<strong>in</strong>azione:<br />

tecnica ed approccio razionale.<br />

The actual experience come out from a practical need which has been made<br />

evident dur<strong>in</strong>g scleros<strong>in</strong>g therapy of reticular ve<strong>in</strong>s and of the spider ve<strong>in</strong>s (or<br />

telangectasias): look<strong>in</strong>g at blood vessel <strong>in</strong> a more evident way, <strong>in</strong> order to use<br />

the needle <strong>in</strong> a more precise and safe way.<br />

The idea of us<strong>in</strong>g <strong>in</strong>struments which should allow to have a better vision than<br />

eyesight or magnify<strong>in</strong>g glass, come out from the need to see “more and better”.<br />

In fact the technique of trans-illum<strong>in</strong>ation has been us<strong>in</strong>g for ten years.<br />

For this purpose different <strong>in</strong>struments have been used, which only <strong>in</strong> part met<br />

our needs: a first <strong>in</strong>strument (figment of our test<strong>in</strong>g!) had a strong lum<strong>in</strong>ous<br />

<strong>in</strong>tensity, but a wavelength which didn’t detect <strong>in</strong> a more clear way the blood<br />

vessel to be sclerosed; <strong>in</strong> addiction, manipulation was still not ergonomic. A<br />

second tool used, worked with a complex system of <strong>in</strong>frared rays with digital<br />

elaboration of pictures which made usage not pratical; however this <strong>in</strong>strument<br />

was too much expensive. A third <strong>in</strong>strument used a led light and was very easy<br />

for the usage, but had a wavelength and a lum<strong>in</strong>ous <strong>in</strong>tensity still not perfect.<br />

A fourth <strong>in</strong>strument lighted up blood vessels <strong>in</strong> a more evident way; bas<strong>in</strong>g on<br />

this, one, it was considered perfect for <strong>in</strong>troduc<strong>in</strong>g the needle with more precision,<br />

but the particular shape obstructed a complete view of ve<strong>in</strong>s picture, made<br />

of reticular ve<strong>in</strong>s and telangectasias.<br />

Therefore, the need to f<strong>in</strong>d a system, which should overcame the difficulties,<br />

brought us day by day to create a new <strong>in</strong>strument, the VISIOVEN®, which is,<br />

<strong>in</strong> our op<strong>in</strong>ion, the most efficient and suitable for our goals. These characteristics<br />

are ma<strong>in</strong>ly due to:<br />

• the particular wavelength of laser light, which easier passes <strong>in</strong>to subcutaneous<br />

tissue and then glance also versus the surface, like a mirror;<br />

• the extreme manageability of handl<strong>in</strong>g, which allowed us to have a<br />

complete view of the system of reticular varices and of telangectasias,<br />

sometimes a wonderful vision!<br />

However, dur<strong>in</strong>g our experience, we found that the trans-illum<strong>in</strong>ation could be<br />

used not only as an <strong>in</strong>strument for more precise detectability of an high number<br />

of ve<strong>in</strong>s, with<strong>in</strong> the scope to proceed “from big to little”, from up versus down,<br />

first “mother ve<strong>in</strong>”, then telangectasias. This procedure, of course, is correct,<br />

but could be changed <strong>in</strong>to a sort of scleros<strong>in</strong>g therapy “a la demand” , prick<strong>in</strong>g<br />

what is possible and visible, based ma<strong>in</strong>ly on morphologic <strong>in</strong>terpretations,<br />

without understand<strong>in</strong>g why scleros<strong>in</strong>g some blood vessel or others.<br />

Otherwise there are not many studies which help us to understand the flows<br />

and refluxes of ve<strong>in</strong> system, which rema<strong>in</strong>s often not visible also at ultrasonography.<br />

This th<strong>in</strong>k<strong>in</strong>g pushed us to beg<strong>in</strong> a speculative way which could br<strong>in</strong>g to give an<br />

hemodynamic mean<strong>in</strong>g to telangiectasias and the reticular system too. First<br />

results are very encourag<strong>in</strong>g and, if they will be confirmed by test<strong>in</strong>g experience<br />

of the <strong>in</strong>volved colleagues, this could represent an unexpected success.<br />

We believe that the pictures have a more strength than speculative reflections<br />

<strong>in</strong>to show<strong>in</strong>g our idea and the results of an easy technique for the execution of<br />

a technique of irreplaceable helpfulness dur<strong>in</strong>g our cl<strong>in</strong>ical practice.<br />

Il nostro lavoro nasce dall’esigenza di rendere più agevole e più razionale la<br />

scleroterapia delle varici di piccolo calibro, <strong>in</strong> particolare Teleangectasie e<br />
