3 - I trattamenti endovascolari<br />
CONCLUSIONI:<br />
Lo scarso follow-up della pubblicazione di Christenson comporta una<br />
deviazione a destra del diamante meta analitico, che, tuttavia, non determ<strong>in</strong>a,<br />
nella valutazione matematica, una dim<strong>in</strong>uzione dell’efficacia del trattamento<br />
LASER rispetto al trattamento chirurgico.<br />
Il trattamento LASER, da questi dati prelim<strong>in</strong>ari meta analitici, appare una<br />
procedura più efficace del trattamento chirurgico nel ridurre la s<strong>in</strong>tomatologia<br />
connessa alla <strong>in</strong>sufficienza venosa cronica degli arti <strong>in</strong>feriori con <strong>in</strong>cont<strong>in</strong>enza<br />
della Vena Grande Safena.<br />
I dati potrebbero essere <strong>in</strong>ficiati dal breve follow-up, dovuto al fatto che il<br />
trattamento LASER endovenoso è di più recente <strong>in</strong>troduzione nel bagaglio<br />
terapeutico per il trattamento della <strong>in</strong>cont<strong>in</strong>enza della vena grande safena.<br />
Considerando che recidive (cl<strong>in</strong>iche, morfo-funzionali, strumentali) dopo<br />
trattamento della <strong>in</strong>sufficienza venosa cronica degli arti <strong>in</strong>feriori con tecnica<br />
chirurgica possono presentarsi anche a distanza di molti anni dalla procedura<br />
stessa, è evidente che più prolungati follow-up (almeno di 5 anni) sono<br />
necessari per poter operare una stima matematica più attendibile del confronto<br />
tra le citate tecniche.<br />
In aggiunta, i lavori analizzati, seppure risultano essere, per impostazione e<br />
numero di pazienti confrontati, i più significativi ed utili ai f<strong>in</strong>i di una valutazione<br />
meta-analitica, sono comunque <strong>in</strong>ficiati dal fatto di non considerare, almeno<br />
nella gran parte di essi, un confronto tra trattamento LASER e s<strong>in</strong>gola metodica<br />
chirurgica. Poichè esistono differenze tra i risultati ottenibili a breve, medio<br />
e lungo term<strong>in</strong>e tra differenti procedure chirurgiche, oggetto di altri lavori<br />
e valutazioni meta-analitiche pubblicate ed <strong>in</strong> considerazione del fatto che<br />
<strong>in</strong>cont<strong>in</strong>enze valvolari ostiali, preostiali, tronculari o comb<strong>in</strong>ate della vena<br />
grande safena potrebbero comportare differenti <strong>in</strong>cidenze di recidive cl<strong>in</strong>icostrumentali<br />
della <strong>in</strong>sufficienza venosa cronica con <strong>in</strong>cont<strong>in</strong>enza della vena<br />
grande safena a secondo della associazione con <strong>in</strong>cont<strong>in</strong>enza del circolo venoso<br />
profondo del tipo di trattamento chirurgico eseguito, è evidente che sono molte<br />
le variabili che andrebbero considerate nei lavori concernenti i risultati di un<br />
trattamento della <strong>in</strong>cont<strong>in</strong>enza della vena grande safena e che la valutazione<br />

di queste variabili potrebbe comportare differenti conclusioni meta-analitiche.<br />
Successive valutazioni del nostro studio riguarderanno il confronto con altre<br />
tecniche endovenose (RF vs. LASER vs. Chirurgia).<br />
