A12.5 GENERIC IMPLEMENTING PROCEDURES OUTLINE<br />
See Element B4.9 and B4.10 for further information on development of procedures.<br />
1. HEADER<br />
On the cover page, write the title of the procedure, document code (if any), type of<br />
confidentiality (if any), version number, and validation date. Other information, such as<br />
author(s) and preparation date, reviewer and review date, responsible manager and approval<br />
date, and signatures, you may wish to put on the next page. All subsequent pages have<br />
header: procedure title, performed by, page number, total number, of pages, document code,<br />
version number, and validation date. Optional: type of confidentiality.<br />
2. ENTRY CONDITION<br />
Prominently display on the cover page the entry conditions for use of the procedure, i.e. the<br />
condition indicating that the procedure is to be used (e.g. upon declaration of a general<br />
emergency).<br />
3. RESPONSIBILITY<br />
Prominently display the position or team responsible for completion of the procedure.<br />
4. CAUTIONS<br />
Prominently display the safety steps performed before use of the procedure (e.g. get<br />
permission from control room, ensure valve X is secure), potential hazards (e.g. heat, live<br />
steam, radiation) and protective equipment/measures to be used.<br />
5. LIMITATIONS (optional)<br />
List the limitations of the <strong>method</strong> or technique used.<br />
6. NEEDED BEFORE USE (optional)<br />
List the tools, protective equipment, resources, documents or information needed to use the<br />
procedure, and the source of these items.<br />
7. PURPOSE - CUSTOMER (optional)<br />
List the expected results (outcome) of the procedure and the identity of the customer - who<br />
gets the product.<br />
8. SUMMARY (optional)<br />
For complex procedures, give a short explanation (summary) of the process (<strong>method</strong>,<br />
technique) followed by a discussion of the conditions under which the procedure is most<br />
effective; advise on possible alternatives and some recommendations may also be given.<br />
9. DEFINITIONS (optional)<br />
Give only those definitions that are needed to perform the procedure.<br />
