12.3.2.6. Natalizumab<br />

Positionnement<br />

— Le natalizumab est un anticorps<br />

monoclonal humanisé proposé en<br />

monothérapie dans les formes très<br />

actives de «sclérose en plaques avec<br />

alternance de poussées et de rémissions».<br />

Effets indésirables<br />

— Réactions liées à la perfusion.<br />

— Infections opportunistes.<br />

— Leucoencéphalite multifocale progressive<br />

(rare mais létale).<br />

— Toxicité hépatique.<br />

12.3.2.7. Inhibiteurs du TNF<br />

L’adalimumab, le certolizumab, l’étanercept,<br />

le golimumab et l’infliximab<br />

sont des inhibiteurs du TNF (Tumor<br />

Necrosis Factor), une cytokine impliquée<br />

dans les processus inflammatoires.<br />

Ils exercent un effet immunomodulateur<br />

prononcé.<br />

Positionnement<br />

— Les inhibiteurs du TNF ne sont en<br />

principe utilisés que dans des affections<br />

évolutives graves, après échec<br />

des traitements classiques (par le<br />

méthotrexate p. ex.) et dans certains<br />

cas, en association, au méthotrexate<br />

p. ex.<br />
